Description
Overview of the medicine
Tezolin is a sustained-release tablet containing Theophylline, used to prevent and treat symptoms of asthma and other lung diseases such as COPD, by relaxing the airways and making breathing easier.
Uses & Indications
Dosage
Adults
Individualized based on serum theophylline levels, typically 200-400 mg twice daily for sustained-release formulations. Max 600 mg/day.
Elderly
Lower doses may be required due to decreased clearance.
Renal_impairment
No dosage adjustment needed for renal impairment, but caution with severe liver impairment.
How to Take
Take orally, usually twice daily, with or without food. Swallow whole, do not crush or chew sustained-release tablets.
Mechanism of Action
Theophylline works by relaxing the smooth muscles of the bronchi and blood vessels in the lungs. It inhibits phosphodiesterase, leading to an increase in intracellular cAMP, which results in bronchodilation. It also has anti-inflammatory effects and can improve diaphragmatic contractility.
Pharmacokinetics
Onset
Typically 1-2 hours for sustained-release formulations.
Excretion
Mainly excreted by the kidneys, with a small portion unchanged in urine.
Half life
Variable, ranging from 7-9 hours in adults, shorter in children and smokers, longer in elderly and those with liver impairment.
Absorption
Well absorbed from the gastrointestinal tract, though absorption rate can vary with sustained-release formulations.
Metabolism
Extensively metabolized in the liver, primarily by CYP1A2 and CYP2E1 enzymes.
Side Effects
Contraindications
- Hypersensitivity to theophylline or other xanthine derivatives
- Acute myocardial infarction
- Severe uncorrected arrhythmias
Drug Interactions
Phenytoin, Phenobarbital, Rifampin
May decrease theophylline levels, reducing efficacy.
Cimetidine, Erythromycin, Ciprofloxacin
May increase theophylline levels, leading to toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, restlessness, tremors, tachycardia, and seizures. Treatment involves discontinuation, activated charcoal, supportive care, and management of symptoms.
Pregnancy & Lactation
Use with caution during pregnancy, only if clearly needed. Theophylline is excreted into breast milk, monitor infants for signs of toxicity.
