Overview of the medicine
Thiopental Sodium is a rapid-acting barbiturate general anesthetic for intravenous administration. It is used for the induction of general anesthesia, for the control of convulsive states (e.g., status epilepticus), and for narcoanalysis.
Uses & Indications
Dosage
Adults
For induction of anesthesia: 3-5 mg/kg intravenously. For convulsive states: 75-125 mg intravenously as a single dose.
Elderly
Reduced doses may be required; monitor closely due to potential for increased sensitivity and prolonged effects.
Renal_impairment
Use with caution; dose adjustment may be necessary in severe renal dysfunction, though renal excretion is mainly of inactive metabolites.
How to Take
Administer intravenously slowly, generally into a large vein, to avoid local irritation and minimize respiratory depression. The solution should be freshly prepared.
Mechanism of Action
Thiopental potentiates GABAergic inhibition by binding to the GABA-A receptor complex, thereby increasing the duration of chloride ion channel opening. This results in hyperpolarization of the neuronal membrane and inhibition of neuronal activity.

Side Effects
Contraindications

Drug Interactions
Probenecid

Storage

Overdose
Symptoms include severe respiratory depression, cardiovascular depression (hypotension, shock), prolonged coma, and areflexia. Management involves supportive care, maintaining a patent airway, artificial ventilation, and administration of vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category C. Thiopental crosses the placenta and can cause fetal CNS depression. Use only if potential benefit justifies potential risk to the fetus. Excreted in breast milk; advise mothers to interrupt breastfeeding for 24 hours after administration.
