Description
Overview of the medicine
Thiopental Sodium is an ultra-short-acting barbiturate used as an intravenous general anesthetic for induction of anesthesia, for control of convulsions, and to reduce elevated intracranial pressure.
Uses & Indications
Dosage
Adults
Induction of Anesthesia: 3-5 mg/kg IV administered over 10-20 seconds. Repeat doses may be given as required. Status Epilepticus: 75-125 mg IV, may repeat in 10-15 minutes if necessary.
Elderly
Reduced doses (typically 30-50% less) are often required due to decreased physiological reserves and slower metabolism.
Renal_impairment
No specific dose adjustment for mild to moderate renal impairment, but caution is advised. Metabolites are renally excreted.
How to Take
Administer intravenously only by trained personnel. Dilute the powder with sterile water for injection or normal saline to a 2.5% or 5% solution. Administer slowly, monitoring vital signs.
Mechanism of Action
Thiopental binds to GABA-A receptors, enhancing the inhibitory effects of GABA. It increases the duration of chloride channel opening, leading to hyperpolarization of neuronal membranes and decreased neuronal excitability, thus causing central nervous system depression.
Pharmacokinetics
Onset
30-60 seconds after intravenous administration.
Excretion
Mainly renal, as inactive metabolites.
Half life
Distribution half-life is 5-10 minutes (due to redistribution); elimination half-life is 11-12 hours (variable, 3-8 hours in some studies).
Absorption
Rapid and complete absorption following intravenous administration, crosses blood-brain barrier quickly.
Metabolism
Primarily hepatic, via oxidation, desulfuration, and N-dealkylation by cytochrome P450 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to barbiturates or any component.
- •Porphyria (latent or manifest).
- •Severe respiratory obstruction or compromised airway.
- •Severe cardiovascular disease (e.g., severe hypotension, shock).
- •Status asthmaticus.
Drug Interactions
Phenytoin
Thiopental may alter phenytoin levels.
Probenecid
May prolong effects of thiopental.
Antihypertensives
May increase hypotensive effects.
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
Potentiates CNS depression, increasing risk of respiratory depression and sedation.
Storage
Store dry powder at controlled room temperature (20-25°C), protected from light. Reconstituted solutions should be stored refrigerated (2-8°C) and used within their specified shelf-life.
Overdose
Symptoms: Severe respiratory depression, apnea, cardiovascular depression (hypotension, shock, cardiac arrest). Management: Maintain patent airway, assist ventilation, administer oxygen. Support cardiovascular function with IV fluids, vasopressors. Closely monitor vital signs. No specific antidote.
Pregnancy & Lactation
Pregnancy Category C/D (depending on source and duration of use). Crosses the placenta and may cause neonatal depression. Use with caution in pregnant women only if potential benefits outweigh risks. Excreted in breast milk; breastfeeding is generally not recommended for 24 hours after administration.
