Description
Overview of the medicine
Thiotab 75 mg Tablet contains Thioctic Acid, also known as Alpha-Lipoic Acid, a potent antioxidant used in the management of peripheral neuropathy, particularly diabetic neuropathy. It helps to improve nerve function and reduce symptoms like pain, numbness, and burning sensations.
Uses & Indications
Dosage
Adults
For diabetic neuropathy: Initially 600 mg orally once daily for 3-4 weeks, then a maintenance dose, possibly 75-150 mg daily as directed by physician. For Thiotab 75 mg specifically: Usually 1-2 tablets daily or as prescribed.
Elderly
No specific dose adjustment required, but use with caution and monitor for side effects.
Renal_impairment
No specific dose adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally, preferably on an empty stomach (e.g., 30 minutes before breakfast) for better absorption. Swallow whole with water.
Mechanism of Action
Thioctic Acid acts as a powerful antioxidant by scavenging free radicals and regenerating other antioxidants like Vitamin C and E. It also acts as a cofactor in mitochondrial enzyme complexes involved in energy metabolism, improving nerve cell function and microcirculation, thereby alleviating neuropathic symptoms.
Pharmacokinetics
Onset
Clinical effects seen after several weeks of continuous treatment.
Excretion
Primarily excreted renally as metabolites.
Half life
Approximately 20-30 minutes for the parent compound.
Absorption
Well absorbed orally, but bioavailability can be variable (approx. 30-60%). Food intake may reduce absorption.
Metabolism
Extensive first-pass metabolism in the liver via beta-oxidation and S-methylation.
Side Effects
Contraindications
- Known hypersensitivity to Thioctic Acid or any excipients
- Children and adolescents due to insufficient data
Drug Interactions
Cisplatin
May reduce the efficacy of cisplatin.
Insulin and Oral Antidiabetics
May potentiate the blood sugar lowering effect, requiring dose adjustment of antidiabetic drugs.
Metal compounds (e.g., iron, magnesium, dairy products)
May chelate with thioctic acid, reducing absorption. Administer at separate times.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, headache, and in severe cases, seizures and lactic acidosis. Management is symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Use during pregnancy and lactation is not recommended unless clearly necessary and advised by a physician, due to insufficient data. Benefits must outweigh potential risks.
