Description
Overview of the medicine
Teneligliptin is an oral antidiabetic drug used for the treatment of type 2 diabetes mellitus. It belongs to the dipeptidyl peptidase-4 (DPP-4) inhibitor class, which works by increasing incretin levels in the body.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily (i.e., two 10 mg tablets). Dosage may be adjusted based on individual patient response and tolerance.
Elderly
No dose adjustment generally required, but caution is advised.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe renal impairment or end-stage renal disease (ESRD), caution is advised.
How to Take
Orally, once daily, with or without food.
Mechanism of Action
Teneligliptin inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which is responsible for inactivating incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, Teneligliptin increases the circulating levels of active incretin hormones, which, in turn, enhance glucose-dependent insulin secretion from pancreatic beta cells and suppress glucagon secretion from pancreatic alpha cells. This leads to improved glycemic control.

Onset
Within hours of administration, significant DPP-4 inhibition observed.
Excretion
Excreted both renally (about 30-50%) and hepatically (about 60-70%), predominantly as metabolites.
Half life
Approximately 15 hours.
Absorption
Rapidly absorbed. Peak plasma concentration (Tmax) is reached in about 1-2 hours. Bioavailability is high (approx. 60-70%).
Metabolism
Primarily metabolized by CYP3A4 and flavin-containing monooxygenase (FMO1 and FMO3) in the liver.
