Description
Overview of the medicine
Tigover 5 mg tablet is a non-steroidal anti-inflammatory drug (NSAID) primarily used for symptomatic relief in various inflammatory and painful conditions. It works by reducing pain, swelling, and inflammation.
Uses & Indications
Dosage
Adults
5 mg once daily, or 2.5 mg twice daily, adjusted based on patient response and tolerability, up to a maximum of 10 mg/day.
Elderly
Initiate with the lowest effective dose (e.g., 2.5 mg once daily) and monitor renal function closely.
Renal_impairment
Use with caution in mild to moderate impairment; dose reduction may be necessary. Not recommended in severe renal impairment.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Tigover selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces prostaglandin production, thereby exerting its anti-inflammatory and analgesic effects.
Pharmacokinetics
Onset
Within 30-60 minutes
Excretion
Mainly excreted via urine (approximately 70%) and feces (approximately 30%) as metabolites.
Half life
Approximately 8-12 hours
Absorption
Rapidly absorbed from the gastrointestinal tract, reaching peak plasma concentrations within 1-2 hours after oral administration.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (e.g., CYP2C9 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Tigover or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- Severe heart failure
- Severe renal or hepatic impairment
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery (peri-operative pain)
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Diuretics
Reduced diuretic and antihypertensive effects, increased risk of renal impairment.
Methotrexate
Increased plasma methotrexate levels, leading to toxicity.
Warfarin, other anticoagulants
Increased risk of bleeding.
ACE inhibitors, Angiotensin II receptor blockers (ARBs)
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management is symptomatic and supportive; gastric lavage and activated charcoal may be considered within 1 hour of ingestion. Monitor vital signs and provide supportive care.
Pregnancy & Lactation
Not recommended during the third trimester of pregnancy due to potential fetal harm (premature closure of ductus arteriosus). Use with caution in first and second trimesters only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
