Description
Overview of the medicine
Timodrop is an ophthalmic solution containing Timolol Maleate, a beta-blocker, used to reduce elevated intraocular pressure in patients with open-angle glaucoma or ocular hypertension.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) once or twice daily.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, and instill the drop. Close eye gently and apply pressure to the corner of the eye near the nose for 1-2 minutes.
Mechanism of Action
Timolol is a non-selective beta-adrenergic receptor blocking agent. It reduces elevated and normal intraocular pressure, possibly by decreasing aqueous humor production or increasing its outflow.
Pharmacokinetics
Onset
30 minutes (IOP reduction)
Excretion
Renal (primarily as metabolites)
Half life
Approximately 2-4 hours (systemic)
Absorption
Systemic absorption occurs after ocular administration, but typically minimal.
Metabolism
Hepatic (extensive first-pass)
Side Effects
Contraindications
- Bronchial asthma or history of bronchial asthma.
- Severe chronic obstructive pulmonary disease (COPD).
- Sinus bradycardia, second or third degree atrioventricular block, overt cardiac failure, cardiogenic shock.
Drug Interactions
Oral beta-blockers
May potentiate systemic beta-blockade.
Calcium channel blockers
Risk of hypotension and/or bradycardia.
Catecholamine-depleting drugs (e.g., Reserpine)
Additive hypotensive and/or bradycardic effects.
Storage
Store at controlled room temperature (15-30°C). Protect from light. Do not freeze.
Overdose
Systemic overdose symptoms include bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Category C in pregnancy. Use only if potential benefit justifies potential risk to the fetus. Small amounts excreted in breast milk, caution advised.

WHO Essential Medicine
YesClinical Trials
Extensively studied in clinical trials for efficacy and safety in reducing intraocular pressure.
Lab Monitoring
- Routine intraocular pressure monitoring. Regular cardiac and pulmonary monitoring for patients with systemic risk factors.
Doctor Notes
- Remind patients about proper administration technique to minimize systemic absorption.
- Assess for pre-existing cardiac or respiratory conditions before prescribing.
- Monitor IOP regularly and evaluate for treatment efficacy.
Patient Guidelines
- Do not touch the dropper tip to any surface to avoid contamination.
- Remove contact lenses before use and wait at least 15 minutes before reinserting.
- Inform your doctor about all other medications you are taking, especially other eye drops or oral beta-blockers.
Missed Dose Advice
If a dose is missed, apply it as soon as possible. If it is almost time for the next dose, skip the missed dose and return to the regular schedule. Do not double the dose.
