Description
Overview of the medicine
Timomin is an ophthalmic solution containing Timolol Maleate, a non-selective beta-adrenergic receptor blocking agent. It is used to lower elevated intraocular pressure in patients with ocular hypertension or chronic open-angle glaucoma.
Uses & Indications
Dosage
Adults
The usual starting dose is one drop of 0.25% Timomin in the affected eye(s) twice a day. If the clinical response is not adequate, the dosage may be changed to one drop of 0.5% Timomin in the affected eye(s) twice a day.
Elderly
No specific dose adjustment is required for elderly patients.
Renal_impairment
No specific dose adjustment is required for renal impairment, but caution is advised with severe impairment due to systemic absorption.
How to Take
Instill one drop in the conjunctival sac of the affected eye(s). Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
Timolol Maleate reduces elevated and normal intraocular pressure, whether or not associated with glaucoma. It primarily reduces aqueous humor production, though some increase in outflow facility has been reported.
Pharmacokinetics
Onset
Onset of action within 30 minutes, peak effect at 1-2 hours.
Excretion
Excreted renally as metabolites and unchanged drug.
Half life
Approximately 4 hours systemically.
Absorption
Systemic absorption occurs from the eye, peak plasma concentrations typically within 1 hour.
Metabolism
Metabolized in the liver.
Side Effects
Contraindications
- •Bronchial asthma or a history of bronchial asthma.
- •Severe chronic obstructive pulmonary disease (COPD).
- •Sinus bradycardia, second or third degree atrioventricular block, overt cardiac failure, cardiogenic shock.
- •Hypersensitivity to Timolol Maleate or any component of the formulation.
Drug Interactions
Oral Beta-blockers
Increased risk of systemic beta-blockade.
Oral Calcium Channel Blockers
Potentiation of AV conduction disturbances and hypotension.
Catecholamine-depleting drugs (e.g., Reserpine)
Possible additive effects, resulting in hypotension and/or marked bradycardia.
Storage
Store at controlled room temperature (20-25°C), protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose may lead to systemic beta-blocker effects such as bradycardia, hypotension, bronchospasm, and acute cardiac failure. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Timolol is excreted in human milk. Exercise caution when administered to a nursing mother.
