Tinabil 20 mg Tablet is a lipid-lowering medication used to reduce cholesterol and triglyceride levels in the blood. It belongs to a class of drugs called statins, which work by inhibiting an enzyme in the liver responsible for cholesterol production. It is used in conjunction with diet and exercise to prevent cardiovascular diseases.
Uses & Indications
Dosage
Adults
The usual starting dose is 10-20 mg once daily. For primary hypercholesterolemia or mixed dyslipidemia, the typical dose is 20 mg once daily. The maximum dose is 40 mg once daily.
Elderly
No dosage adjustment required for elderly patients with normal renal function. A starting dose of 5 mg once daily may be considered for patients over 70 years.
Renal_impairment
For patients with moderate renal impairment (creatinine clearance <60 mL/min), a starting dose of 5 mg once daily is recommended. The 40 mg dose is contraindicated in severe renal impairment.
How to Take
Tinabil tablets should be swallowed whole with water, once daily, with or without food. It is generally recommended to take the dose at the same time each day, preferably in the evening.
Mechanism of Action
Tinabil selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early, rate-limiting step in cholesterol biosynthesis. This inhibition reduces the production of mevalonate, a precursor to cholesterol. This results in a decrease in hepatic cholesterol synthesis and an increase in the number of hepatic LDL receptors on the cell surface, enhancing uptake and catabolism of LDL. It also reduces VLDL and triglyceride levels.

Cholesterol-lowering effect typically starts within 1-2 weeks; maximal effect within 2-4 weeks.
Excretion
Mainly excreted in the feces via biliary route; a small portion is excreted in urine.

Approximately 19 hours.

Rapidly absorbed from the GI tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver, mainly by CYP2C9 and CYP3A4, to active and inactive metabolites.

Contraindications
- Hypersensitivity to Tinabil or any component of the tablet.
- Active liver disease, including unexplained persistent elevations of serum transaminases.
- Pregnancy and lactation.
- Severe renal impairment (creatinine clearance <30 mL/min).
- Concomitant use of cyclosporine.
