Overview of the medicine
Metronidazole injection is an antibiotic and antiprotozoal medication used to treat a wide range of bacterial and parasitic infections. It is effective against anaerobic bacteria and certain parasites like amoebae and trichomonads.
Uses & Indications
Dosage
Adults
For most anaerobic infections: 500 mg every 8 hours by slow intravenous infusion (over 30-60 minutes). Surgical prophylaxis: 500 mg as a single dose prior to surgery.
Elderly
No specific dosage adjustment is generally required for elderly patients unless there is severe hepatic impairment.
Renal_impairment
No dose adjustment is usually needed for mild to moderate renal impairment. For severe renal impairment (CrCl <10 mL/min) or patients on hemodialysis, consider reducing the dose or extending the dosing interval.
How to Take
Tinimet 5 mg Injection should be administered by slow intravenous infusion over a period of 30 to 60 minutes. Do not administer as a bolus injection. Always follow healthcare provider's instructions.
Mechanism of Action
Metronidazole enters microbial cells, where it is reduced by electron-transport proteins to form highly reactive nitro-free radicals. These radicals disrupt DNA helical structure, inhibit nucleic acid synthesis, and ultimately lead to microbial cell death.

Side Effects
Contraindications
- •Hypersensitivity to metronidazole, other nitroimidazole derivatives, or any component of the formulation.

Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia, shortness of breath). Avoid alcohol during and for at least 3 days after treatment.
Lithium
May increase serum lithium levels, leading to lithium toxicity. Monitor lithium levels.
Warfarin
Potentiates the anticoagulant effect, increasing risk of bleeding. Monitor INR closely.
Cimetidine
May increase metronidazole serum levels. Adjust metronidazole dose as needed.
Phenytoin/Phenobarbital
May decrease metronidazole serum levels. Adjust metronidazole dose as needed.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, and mild disorientation. There is no specific antidote. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B/C (varies by trimester and source). Metronidazole crosses the placenta. It is generally avoided in the first trimester. Excreted into breast milk; caution advised, and a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
