Tivanik 250 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days OR 500 mg as a single dose on day 1, followed by 250 mg once daily for 4 days. For STIs, a single 1g dose (four 250mg tablets) is common.
Elderly
No specific dosage adjustment is recommended for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for mild to moderate renal impairment (creatinine clearance >40 mL/min). Data is limited for severe impairment.
How to Take
Tivanik tablets can be taken with or without food. However, taking it with food may help reduce gastrointestinal upset. Swallow the tablet whole with water.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms and, thus, inhibits protein synthesis. This results in the inhibition of the bacterium's growth and reproduction.

Onset
Peak plasma concentrations are generally achieved within 2 to 3 hours.
Excretion
Mainly excreted unchanged in the bile; a small proportion (around 6%) is excreted renally.
Half life
Prolonged terminal elimination half-life of 2 to 4 days, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; however, bioavailability is approximately 37% due to significant first-pass metabolism.

Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Antacids
Concurrent use of antacids containing aluminum or magnesium may reduce the peak serum concentration of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin concentrations. Adjust azithromycin dose.
QT-prolonging drugs
Increased risk of cardiac arrhythmias. Use with caution.
