Description
Overview of the medicine
Tixol is a combination medicine containing Cinnarizine and Dimenhydrinate. It is primarily used for the symptomatic treatment of vertigo of various origins, including Meniere's disease and motion sickness. Cinnarizine acts as an antihistamine and calcium channel blocker, while Dimenhydrinate is an antihistamine with antiemetic properties.
Uses & Indications
Dosage
Adults
1 tablet (Cinnarizine 20 mg + Dimenhydrinate 40 mg) two to three times daily, or as directed by physician.
Elderly
Same as adults, but caution should be exercised and dosage adjustment may be considered in patients with impaired renal or hepatic function.
Renal_impairment
Use with caution. Dosage adjustment may be necessary based on the degree of impairment.
How to Take
For oral use. Tablets should be taken after meals with some liquid to minimize gastrointestinal irritation.
Mechanism of Action
Cinnarizine is an antihistamine and a calcium channel blocker. It inhibits the influx of calcium into vestibular sensory cells, thereby reducing vertigo symptoms. Dimenhydrinate is also an antihistamine with significant antiemetic and anticholinergic properties, reducing nausea and vomiting associated with vertigo and motion sickness. The combination provides a synergistic effect in managing vestibular disorders.
Pharmacokinetics
Onset
Cinnarizine: 1-3 hours (peak effect); Dimenhydrinate: 15-30 minutes.
Excretion
Cinnarizine is excreted about two-thirds via faeces and one-third via urine. Dimenhydrinate and its metabolites are primarily excreted in the urine.
Half life
Cinnarizine: approximately 4 hours; Dimenhydrinate (Diphenhydramine component): 3.5-9 hours.
Absorption
Both Cinnarizine and Dimenhydrinate are well and rapidly absorbed from the gastrointestinal tract. Cinnarizine reaches peak plasma levels in 1-3 hours, while Dimenhydrinate acts within 15-30 minutes.
Metabolism
Both drugs undergo extensive hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Cinnarizine, Dimenhydrinate, or other antihistamines
- Acute porphyria
- Known history of glaucoma
- Prostatic hypertrophy
- Children under 18 years (due to lack of safety and efficacy data for this combination in this age group)
Drug Interactions
Anticholinergic drugs
Potentiation of anticholinergic side effects (e.g., dry mouth, urinary retention).
Ototoxic drugs (e.g., aminoglycosides)
May mask symptoms of ototoxicity.
CNS depressants (e.g., alcohol, hypnotics, sedatives)
Increased sedative effects.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, ataxia, CNS depression (sedation, coma), anticholinergic effects (dry mouth, dilated pupils, tachycardia, urinary retention), and convulsions. Management is supportive, including gastric lavage, activated charcoal, and symptomatic treatment.
Pregnancy & Lactation
Not recommended during pregnancy and lactation due to insufficient data and potential risks. Use only if potential benefit justifies the potential risk to the fetus or infant, and under strict medical supervision.

Clinical Trials
Clinical trials have shown the combination of Cinnarizine and Dimenhydrinate to be more effective than either drug alone in treating vertigo, particularly in acute vestibular syndrome.
Lab Monitoring
- No specific routine lab monitoring is generally required for healthy individuals. Monitor liver and renal function in patients with pre-existing impairment.
Doctor Notes
- Caution in elderly patients, especially those with pre-existing neurological conditions due to increased risk of extrapyramidal symptoms.
- Advise patients about potential drowsiness and to avoid activities requiring mental alertness.
- Monitor for anticholinergic side effects in susceptible patients.
Patient Guidelines
- Do not drive or operate heavy machinery due to potential drowsiness.
- Avoid alcohol consumption.
- Take medicine after meals to reduce stomach upset.
- Inform your doctor if symptoms do not improve or worsen.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
