Description
Overview of the medicine
Tobidex eye drop is a combination medicine containing an antibiotic (Tobramycin) and a corticosteroid (Dexamethasone). It is used to treat bacterial eye infections where inflammation is also present, providing both infection control and inflammation reduction.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the affected eye(s) every 4-6 hours. For severe cases, dosage may be increased to 1-2 drops every 2 hours for the first 24-48 hours, then decreased gradually.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for renal impairment, as systemic absorption is minimal.
How to Take
For ophthalmic use only. Shake well before use. Do not touch the dropper tip to any surface to avoid contamination. Remove contact lenses before administration and wait at least 15 minutes before reinserting.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that works by inhibiting bacterial protein synthesis, leading to bacterial cell death. Dexamethasone is a potent corticosteroid that suppresses the inflammatory response by inhibiting various inflammatory mediators, thereby reducing swelling, redness, and pain.
Pharmacokinetics
Onset
Anti-inflammatory effects of dexamethasone can be observed within hours. Antibacterial effects of tobramycin begin rapidly upon contact with susceptible bacteria.
Excretion
Systemic excretion is negligible due to limited absorption from the eye surface.
Half life
Not clinically significant for ophthalmic use due to minimal systemic absorption.
Absorption
Systemic absorption of both tobramycin and dexamethasone from ophthalmic administration is minimal. High local concentrations are achieved in ocular tissues.
Metabolism
Minimal systemic metabolism occurs after ophthalmic administration.
Side Effects
Contraindications
- Hypersensitivity to tobramycin, dexamethasone, or any component of the formulation.
- Active viral infections of the cornea and conjunctiva (e.g., epithelial herpes simplex keratitis, vaccinia, varicella).
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
- Untreated parasitic eye infections.
Drug Interactions
Systemic corticosteroids
Although systemic absorption is minimal, concurrent use of systemic corticosteroids may potentially increase systemic side effects, especially with prolonged use.
Other topical aminoglycosides
Concurrent use may increase the risk of local hypersensitivity reactions.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with ophthalmic administration. If accidental ingestion occurs, seek medical advice. Topical overdose may be flushed from the eye(s) with lukewarm water.
Pregnancy & Lactation
Use in pregnancy and lactation only if the potential benefit outweighs the potential risk to the fetus or infant. Category C. Consult a doctor.

The combination of tobramycin and dexamethasone has been extensively studied and is a well-established treatment for ocular infections with inflammation. Numerous clinical trials support its efficacy and safety.
Lab Monitoring
- Intraocular pressure (IOP) should be routinely monitored, especially with prolonged use (more than 10 days).
Doctor Notes
- Monitor intraocular pressure regularly, particularly in patients with pre-existing glaucoma or a family history of glaucoma.
- Discontinue if fungal infection is suspected.
- Avoid prolonged use to minimize steroid-related complications.
Patient Guidelines
- Follow your doctor's instructions carefully regarding dosage and duration.
- Do not wear contact lenses during treatment unless advised by your doctor.
- Do not share your eye drops with others.
- Complete the full course of treatment even if symptoms improve.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not apply a double dose to make up for a missed one.
