Description
Overview of the medicine
Tobidex Eye Ointment is a combination of an antibiotic (Tobramycin) and a corticosteroid (Dexamethasone) used to treat ocular inflammatory conditions where bacterial infection or risk of bacterial infection exists.
Uses & Indications
Dosage
Adults
Apply a small amount (approximately a 1/2 inch ribbon) into the conjunctival sac three to four times daily.
Elderly
Same as adult dosage. No specific dosage adjustment is generally required.
Renal_impairment
No specific dosage adjustment required due to minimal systemic absorption from ophthalmic use.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch. Apply a small ribbon of ointment into the pouch. Close eye gently for 1-2 minutes. Do not touch the tip of the tube to the eye or any other surface.
Mechanism of Action
Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis, leading to bactericidal effects. Dexamethasone is a potent corticosteroid that suppresses the inflammatory response by inhibiting multiple inflammatory mediators, thereby reducing swelling, redness, and itching.
Pharmacokinetics
Onset
Anti-inflammatory effects of Dexamethasone can be observed within hours; complete antibiotic effect on infection may take several days.
Excretion
Absorbed Tobramycin is primarily excreted renally; Dexamethasone metabolites are excreted via hepatic pathways.
Half life
Systemic half-life not clinically relevant due to minimal ophthalmic absorption.
Absorption
Minimal systemic absorption occurs following ophthalmic administration, thus systemic effects are generally negligible.
Metabolism
Dexamethasone is metabolized in the liver; Tobramycin is primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to any component of the medication.
- Epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, varicella, and many other viral diseases of the cornea and conjunctiva.
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
- Untreated purulent infection of the eye.
Drug Interactions
Systemic Drugs
No significant systemic drug interactions are expected due to minimal ocular absorption.
Other Ophthalmic Preparations
If using other eye drops or ointments, administer them at least 5-10 minutes apart.
Storage
Store at room temperature (15-30°C), protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with ophthalmic administration. If irritation or excessive watering occurs, flush the eye with lukewarm water.
Pregnancy & Lactation
Use during pregnancy or lactation only if the potential benefit justifies the potential risk to the fetus or infant. Consult a doctor.
