Description
Overview of the medicine
Tobramycin is an aminoglycoside antibiotic used to treat bacterial infections of the eye. It is effective against a wide range of gram-negative and some gram-positive bacteria.
Uses & Indications
Dosage
Adults
Mild to moderate infections: 1-2 drops into the affected eye(s) every 4-6 hours. Severe infections: 2 drops into the affected eye(s) every hour until improvement, then reduce frequency.
Elderly
No specific dosage adjustment required.
Renal_impairment
No specific dosage adjustment required for topical ophthalmic use.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, instill the drops into the pouch. Close the eye gently for 1-2 minutes. Avoid touching the dropper tip to any surface.
Mechanism of Action
Tobramycin exerts its bactericidal effect by irreversibly binding to the 30S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis.
Pharmacokinetics
Onset
Rapid onset of action, typically within minutes.
Excretion
Primarily excreted unchanged via the kidneys if absorbed systemically, but local excretion through tears.
Half life
Not significant systemically; locally, drug clears with tear turnover.
Absorption
Minimal systemic absorption following ocular administration.
Metabolism
Not significantly metabolized after topical ocular administration.
Side Effects
Contraindications
- Hypersensitivity to tobramycin or any component of the formulation
- Viral or fungal eye infections
Drug Interactions
Systemic aminoglycosides
Increased risk of ototoxicity/nephrotoxicity if significant systemic absorption occurs (rare with ophthalmic use).
Other ophthalmic medications
If using multiple ophthalmic medications, administer them at least 5 minutes apart.
Storage
Store at room temperature (below 30°C). Do not freeze. Keep out of reach of children.
Overdose
Due to minimal systemic absorption, systemic overdose is unlikely. Local irritation may occur with excessive topical use. Symptomatic and supportive treatment.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Tobramycin is excreted in breast milk; caution advised during lactation.
