Description
Overview of the medicine
Topcef 1 gm Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic, used to treat a wide range of bacterial infections. It is administered parenterally, typically intravenously or intramuscularly, after reconstitution.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2 gm once daily (every 24 hours) or in two equally divided doses (every 12 hours) depending on the type and severity of infection, up to a maximum of 4 gm per day. For uncomplicated gonorrhea, a single 500 mg IM dose is recommended. For surgical prophylaxis, 1 gm IV/IM 30-90 minutes prior to surgery.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function. Dosage should be adjusted based on renal and hepatic impairment if present.
Renal_impairment
No dosage adjustment is necessary for Ceftriaxone in patients with mild to moderate renal impairment. For severe renal impairment (creatinine clearance <10 mL/min), the daily dose should not exceed 2 gm. In dialysis patients, no supplementary dose is needed after dialysis.
How to Take
Administer intramuscularly (IM) or intravenously (IV). For IM administration, reconstitute the powder with sterile water for injection or lidocaine 1%. For IV administration, reconstitute with sterile water for injection and infuse slowly over 30 minutes. Do not mix with calcium-containing solutions.
Mechanism of Action
Ceftriaxone is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the transpeptidation process necessary for peptidoglycan synthesis, thereby disrupting cell wall formation and leading to bacterial lysis.

Excretion
Excreted unchanged via both renal (40-65%) and biliary/fecal (35-60%) routes.
Half life
Approximately 5.8 to 8.7 hours (prolonged in neonates and patients with severe renal/hepatic impairment)
Absorption
Rapidly and completely absorbed after intramuscular (IM) injection; peak plasma concentrations reached within 2-3 hours. Intravenous (IV) administration provides immediate and complete bioavailability.
Metabolism
Not extensively metabolized in the liver; approximately 33-67% is excreted unchanged in urine and bile.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone, any other cephalosporin, or any of the excipients.
- •Patients with previous immediate and severe hypersensitivity to penicillin or any other beta-lactam antibiotic.
- •Neonates (≤28 days) with hyperbilirubinemia, especially premature neonates.
- •Neonates (≤28 days) requiring IV calcium-containing solutions or infusions, due to the risk of precipitation.
Drug Interactions
Aminoglycosides
Physical incompatibility and potential for reduced activity if mixed in the same syringe/solution. Administer separately.
Calcium-containing solutions
Do not mix or co-administer with IV calcium-containing solutions, especially in neonates.
Loop diuretics (e.g., Furosemide)
May increase nephrotoxicity when co-administered, especially in patients with renal impairment.
Oral anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect. INR monitoring is advised.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Reconstituted solution should be used immediately or stored as per manufacturer's guidelines, typically refrigerated for a short period.
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, and convulsions. Treatment is symptomatic and supportive. Ceftriaxone is not significantly removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Ceftriaxone is excreted in low concentrations in breast milk; caution should be exercised when administered to a nursing woman.
