Overview of the medicine
Ceftriaxone is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typically 1-2 g once daily or in two divided doses, up to 4 g/day for severe infections. For 500mg, often 500 mg - 1 g once daily depending on indication.
Elderly
Dosage adjustment may not be necessary unless severe renal/hepatic impairment.
Renal_impairment
No dosage adjustment required for mild to moderate impairment. For severe impairment (CrCl <10 mL/min), dosage may be capped at 2 g/day.
How to Take
For IM injection, administer deep into a large muscle. For IV injection, administer slowly over 2-4 minutes. For IV infusion, administer over 30 minutes.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).

Excretion
Primarily via urine and bile.
Half life
Approximately 5.8 to 8.7 hours.
Absorption
Rapid and complete after IM, immediate after IV.
Metabolism
Partially metabolized to inactive compounds in the intestine.
Side Effects
Contraindications
- •Hypersensitivity to ceftriaxone or other cephalosporins.
- •Neonates with hyperbilirubinemia.
- •Neonates receiving calcium-containing IV solutions.
Drug Interactions
Loop diuretics
Possible increased risk of nephrotoxicity.
Aminoglycosides
Enhanced nephrotoxicity when co-administered.
Oral anticoagulants
May increase the anticoagulant effect.
Calcium-containing solutions
Concurrent administration with IV calcium-containing solutions is contraindicated in neonates.
Storage
Store intact vials below 30°C, protected from light. Do not freeze.
Overdose
Symptomatic and supportive treatment. No specific antidote. Hemodialysis or peritoneal dialysis is not effective.
Pregnancy & Lactation
Pregnancy: Category B. Use only if clearly needed. Lactation: Excreted in breast milk in small amounts. Use with caution.
