Description
Overview of the medicine
Topiderm contains Mometasone Furoate, a potent corticosteroid used to treat various inflammatory skin conditions such as eczema, dermatitis, and psoriasis.
Uses & Indications
Dosage
Adults
Apply a thin film of Topiderm Cream to the affected skin areas once daily.
Elderly
Similar to adult dosage. Caution should be exercised due to potential for thinner skin and increased absorption.
Renal_impairment
No specific dose adjustment required for topical application.
How to Take
For external use only. Apply a thin layer to the affected skin area once daily. Gently rub in. Do not use on the face for prolonged periods without medical supervision. Avoid contact with eyes.
Mechanism of Action
Mometasone Furoate is a synthetic corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties. It acts by inducing phospholipase A2 inhibitory proteins, thereby controlling the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Clinical improvement typically seen within a few days of consistent application.
Excretion
Excreted primarily via urine and feces.
Half life
Not well-defined for topical application; systemic mometasone has a half-life of approximately 5.8 hours.
Absorption
Minimal systemic absorption through intact skin (approximately 0.4% after 8 hours). Absorption increases with occlusion, skin damage, or application to large surface areas.
Metabolism
Primarily metabolized in the liver, with some local metabolism in the skin.
Side Effects
Contraindications
- Hypersensitivity to mometasone furoate or any components of the cream
- Bacterial, fungal, viral or parasitic skin infections (e.g., herpes simplex, varicella)
- Perioral dermatitis
- Rosacea
- Acne vulgaris
- Perianal and genital pruritus
- Ulcerative conditions
- Vaccination reactions
Drug Interactions
0
Systemic absorption is minimal, therefore significant drug interactions are unlikely. However, concurrent use of other topical corticosteroids should be avoided.
1
Topical drugs are generally considered to have a low risk of systemic drug interactions.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Acute overdose is unlikely. However, chronic overuse or misuse may lead to systemic corticosteroid effects, including adrenal suppression. Management involves gradual withdrawal of the drug and supportive therapy.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: It is not known whether topical corticosteroids are excreted in human milk. Use with caution.

Lab Monitoring
- Not generally required for topical use. In cases of extensive or prolonged use, monitoring for systemic corticosteroid effects (e.g., adrenal function tests) may be considered.
- Baseline and periodic skin evaluations for signs of atrophy or infection.
Doctor Notes
- Educate patients on proper application technique and duration of use.
- Advise caution when prescribing for facial use, recommending short courses.
- Be aware of potential for tachyphylaxis and rebound effects with prolonged use.
- Consider alternative non-steroidal options for long-term management where appropriate.
