Trado-100-mg-injection contains Tramadol Hydrochloride, an opioid analgesic used to treat moderate to moderately severe pain. It works by changing how the brain and nervous system respond to pain.
Uses & Indications
Dosage
Adults
Adults: 50-100 mg intravenously or intramuscularly every 4-6 hours as needed. Maximum 400 mg/day.
Elderly
Elderly (>75 years): Reduced dosage may be necessary; consider starting at a lower dose (e.g., 50 mg every 6 hours) and extending dosing intervals, especially with renal impairment. Max 300 mg/day.
Renal_impairment
Creatinine clearance <30 mL/min: 50-100 mg every 12 hours. Not recommended for severe renal impairment.
How to Take
For intravenous (slow injection or infusion) or intramuscular administration. Dilution may be required for intravenous infusion.
Mechanism of Action
Tramadol acts as a weak μ-opioid receptor agonist. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effects.

Excretion
Mainly excreted renally (approximately 90%) and about 10% via feces.
Half life
Parent drug: 5-7 hours; Active metabolite (M1): ~9 hours
Absorption
Rapidly and completely absorbed after intramuscular or intravenous administration. Bioavailability is high.
Metabolism
Extensively metabolized in the liver by CYP2D6 and CYP3A4 to O-desmethyltramadol (M1, active) and other inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to tramadol or any excipient
- •Acute intoxication with alcohol, hypnotics, analgesics, opioids, or other psychotropic drugs
- •Patients receiving MAO inhibitors or within 14 days of discontinuing them
- •Epilepsy not adequately controlled by treatment
- •Severe respiratory depression
Drug Interactions
Carbamazepine
Significant reduction in tramadol analgesic effect and duration of action due to increased metabolism.
MAO Inhibitors
Increased risk of serotonin syndrome and seizures.
Serotonergic drugs (e.g., SSRIs, SNRIs, TCAs)
Increased risk of serotonin syndrome.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased risk of respiratory depression, profound sedation, coma, and death.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms include respiratory depression, seizures, miosis, circulatory collapse, and coma. Treatment involves supportive care, maintaining airway and ventilation, and administration of opioid antagonist naloxone to reverse respiratory depression. Naloxone may not fully reverse seizures.
Pregnancy & Lactation
Pregnancy: Not recommended during pregnancy, especially in the third trimester, due to risk of neonatal opioid withdrawal syndrome. Lactation: Tramadol and its metabolites are excreted in breast milk. Use is not recommended during breastfeeding.
