Overview of the medicine
Trado 50 mg Capsule contains Tramadol Hydrochloride, which is used to relieve moderate to moderately severe pain. It works in the brain to change how your body feels and responds to pain.
Uses & Indications
Dosage
Adults
Oral: 50-100 mg every 4-6 hours as needed, not to exceed 400 mg/day.
Elderly
For patients over 75 years, a reduced maximum daily dose of 300 mg is recommended. Close monitoring for adverse effects.
Renal_impairment
For creatinine clearance <30 mL/min, dosage interval should be increased to every 12 hours, not to exceed 200 mg/day.
How to Take
Trado capsules should be swallowed whole with a sufficient amount of liquid, with or without food. Do not chew or crush.
Mechanism of Action
Tramadol works by acting on opioid receptors in the brain and spinal cord, reducing pain signals. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effect.
Pharmacokinetics
Onset
Analgesic effect begins within 30-60 minutes.
Excretion
Excreted primarily by the kidneys (90%), with 10% excreted in feces.
Half life
Approximately 5-7 hours for the parent drug and 7-9 hours for the active metabolite O-desmethyltramadol.
Absorption
Rapid and almost complete absorption from the gastrointestinal tract. Bioavailability is about 75%.
Metabolism
Extensive hepatic metabolism via N- and O-demethylation (primarily by CYP2D6 and CYP3A4). O-desmethyltramadol is an active metabolite.
Side Effects
Contraindications
- •Hypersensitivity to tramadol or any component of the formulation.
- •Acute intoxication with alcohol, hypnotics, analgesics, opioids, or other psychotropic drugs.
- •Patients receiving monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
- •Severe respiratory depression.
- •Epilepsy not controlled by treatment.
Drug Interactions
Warfarin
Increased INR and risk of bleeding.
Carbamazepine
Significantly decreases tramadol plasma levels, reducing analgesic effect.
MAO Inhibitors
Increased risk of serotonin syndrome and seizures.
SSRIs/SNRIs/TCAs
Increased risk of serotonin syndrome and seizures.
CNS Depressants (e.g., alcohol, benzodiazepines)
Increased risk of respiratory depression, profound sedation, coma, and death.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include respiratory depression, seizures, somnolence, coma, skeletal muscle flaccidity, cold and clammy skin, constricted pupils, and possibly death. Treatment includes supportive measures, administration of naloxone for respiratory depression, and benzodiazepines for seizures.
Pregnancy & Lactation
Not recommended during pregnancy due to the risk of neonatal opioid withdrawal syndrome. Should not be used during lactation as tramadol and its metabolites are excreted in breast milk.
