Description
Overview of the medicine
Tranexamic acid is an antifibrinolytic agent that is used to prevent and treat excessive blood loss in various medical conditions, including heavy menstrual bleeding, surgical procedures, and certain bleeding disorders.
Uses & Indications
Dosage
Adults
For menorrhagia: 1g (2 capsules) three times daily for up to 4 days. For other indications: 500 mg to 1.5 g (1-3 capsules) 2-3 times daily, depending on severity and condition.
Elderly
No specific dose reduction required unless there is renal impairment. Adjust dosage based on renal function.
Renal_impairment
Dosage reduction is necessary. For mild impairment: 500 mg twice daily. For moderate: 500 mg daily. For severe: 500 mg every 48 hours.
How to Take
Take orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Tranexamic acid inhibits fibrinolysis by reversibly binding to plasminogen, thereby preventing its activation to plasmin. This stabilizes fibrin clots, leading to reduced bleeding.
Pharmacokinetics
Onset
Onset of action varies depending on the indication; typically within 1-2 hours for oral capsules.
Excretion
Excreted primarily unchanged in the urine via glomerular filtration.
Half life
Approximately 2 to 11 hours, significantly prolonged in renal impairment.
Absorption
Rapidly absorbed from the GI tract; bioavailability is approximately 30-50% after oral administration.
Metabolism
Minimally metabolized; less than 5% of the dose is metabolized.

- •Active intravascular clotting
- •Subarachnoid hemorrhage (due to risk of cerebral edema and infarction)
