Overview of the medicine
Tranexamic acid is an antifibrinolytic agent used to prevent or reduce excessive bleeding in various medical and surgical conditions.
Uses & Indications
Dosage
Adults
For local fibrinolysis: 500 mg (1 ampoule) by slow intravenous injection 2 to 3 times daily. For general fibrinolysis: 1 g (2 ampoules) by slow intravenous injection 3 times daily. Doses should be adjusted based on the specific indication and patient condition.
Elderly
No specific dose reduction required unless there is renal impairment.
Renal_impairment
Dose reduction is necessary based on creatinine clearance. For CrCl 30-60 mL/min: 10 mg/kg twice daily. For CrCl 10-30 mL/min: 10 mg/kg daily. For CrCl < 10 mL/min: 5 mg/kg daily.
How to Take
Administered by slow intravenous injection or infusion. Must not be given intramuscularly.
Mechanism of Action
Tranexamic acid inhibits fibrinolysis by reversibly blocking the lysine binding sites on plasminogen, thereby preventing plasmin from binding to and degrading fibrin.

Mainly via renal excretion (glomerular filtration) within 24 hours.

Side Effects
Contraindications
- •Active intravascular clotting (e.g., DVT, pulmonary embolism)

- •Subarachnoid hemorrhage (risk of cerebral edema and infarction)
- •Hypersensitivity to tranexamic acid

Increased risk of thromboembolism.
Anticoagulants (e.g., Warfarin, Heparin)
Concurrent use should be avoided as tranexamic acid reverses the anticoagulant effects.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms may include nausea, vomiting, diarrhea, dizziness, headache, hypotension. Management is supportive and symptomatic, including fluid and electrolyte replacement. Hemodialysis may be considered in severe renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; generally considered safe for breastfeeding, but caution is advised.
