Description
Overview of the medicine
Trapic (Tranexamic Acid) is an antifibrinolytic agent used to prevent and treat various types of bleeding. It works by inhibiting the breakdown of blood clots, thus promoting hemostasis.
Uses & Indications
Dosage
Adults
Oral: 1-1.5 g (2-3 tablets of 500 mg) two to three times daily. IV: 10 mg/kg three to four times daily. Dosing varies significantly by indication and route.
Elderly
Generally, no specific dose adjustment needed unless renal impairment is present. Start with lower doses.
Renal_impairment
Dose adjustment required based on creatinine clearance. For severe impairment, not recommended.
How to Take
Tablets can be taken with or without food. Injections are administered slowly intravenously.
Mechanism of Action
Tranexamic acid competitively inhibits the activation of plasminogen to plasmin, thereby preventing the dissolution of fibrin clots. It binds to the lysine binding sites on plasminogen and plasmin, thus blocking their interaction with fibrin.
Pharmacokinetics
Onset
Oral: 1-2 hours; Intravenous: Rapid.
Excretion
Primarily excreted unchanged in the urine, mainly by glomerular filtration, within 24 hours.
Half life
Approximately 2-11 hours (dose and renal function dependent).
Absorption
Rapidly absorbed after oral administration, with bioavailability around 30-50%. Peak plasma concentration is reached within 3 hours.
Metabolism
Minimally metabolized; less than 5% of the dose is metabolized.
Side Effects
Contraindications
- Active intravascular clotting
- Severe renal impairment
- History of thrombosis or thromboembolic disease
- Subarachnoid hemorrhage (risk of cerebral edema and infarction)
- Hypersensitivity to tranexamic acid
Drug Interactions
Combined hormonal contraceptives
Increased risk of thrombosis.
Factor IX complex concentrates or anti-inhibitor coagulant complexes
Increased risk of thrombosis.

Store below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms may include nausea, vomiting, diarrhea, orthostatic hypotension, dizziness, headache, and visual disturbances. Management is supportive, including gastric lavage or induced emesis, and symptomatic treatment.
