Description
Overview of the medicine
Trevox 500 mg Injection is a broad-spectrum antibiotic belonging to the fluoroquinolone class. It is used to treat a wide range of bacterial infections, including those of the respiratory tract, urinary tract, skin, and soft tissues, when oral administration is not feasible or severe infection requires intravenous therapy.
Uses & Indications
Dosage
Adults
500 mg once daily by intravenous infusion over 60 minutes for 7-14 days, depending on the infection.
Elderly
No dose adjustment is generally required unless renal function is impaired.
Renal_impairment
Dose adjustment required based on creatinine clearance: CrCl 20-49 mL/min: 250 mg every 24 hours; CrCl <20 mL/min: 250 mg every 48 hours.
How to Take
Trevox Injection should be administered by slow intravenous infusion over a period of not less than 60 minutes. It should not be administered via rapid intravenous bolus or intramuscularly. Do not mix with other medications.
Mechanism of Action
Trevox acts by inhibiting bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA, ultimately causing bacterial cell death.
Pharmacokinetics
Onset
Within 30-60 minutes after infusion.
Excretion
Primarily excreted unchanged in the urine, with a small portion excreted via feces.
Half life
Approximately 6-8 hours, allowing for once or twice daily dosing.
Absorption
Rapid and complete absorption after intravenous administration, achieving peak plasma concentrations within 1 hour.
Metabolism
Minimally metabolized in the liver (less than 10%), primarily to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Trevox, other fluoroquinolones, or any component of the formulation.
- History of tendon disorders related to fluoroquinolone administration.
- Epilepsy or other seizure disorders.
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered.
Antacids/Sucralfate/Cation-containing products
May reduce absorption when given orally (not applicable for IV, but caution needed if switching to oral).
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
In case of overdose, symptomatic treatment and supportive measures should be initiated. Monitor ECG due to potential for QT prolongation. Hemodialysis or peritoneal dialysis may not be effective in removing Trevox.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
