Description
Overview of the medicine
Trevox 500 mg Tablet is a broad-spectrum antibiotic used to treat various bacterial infections, including respiratory tract infections, urinary tract infections, and skin and soft tissue infections. It belongs to the fluoroquinolone class of antibiotics.
Uses & Indications
Dosage
Adults
500 mg once daily for 7-14 days, depending on the type and severity of infection.
Elderly
No specific dose adjustment is required based on age alone, but renal function should be assessed.
Renal_impairment
Dose adjustment required for patients with creatinine clearance < 50 mL/min. Consult physician for specific recommendations.
How to Take
Take Trevox 500 mg Tablet orally with a glass of water, with or without food. Do not take with antacids containing aluminum or magnesium, dairy products, or iron supplements, as these can reduce absorption. Separate administration by at least 2 hours.
Mechanism of Action
Trevofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately bacterial cell death.
Pharmacokinetics
Onset
Clinical effects typically observed within 1-2 hours of administration.
Excretion
Mainly excreted renally (glomerular filtration and tubular secretion) and to a lesser extent via feces.
Half life
Approximately 8-10 hours, allowing for once-daily dosing.
Absorption
Rapidly and well-absorbed after oral administration, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Limited hepatic metabolism; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to trevofloxacin or other fluoroquinolones.
- History of tendon rupture associated with fluoroquinolone use.
- Patients with epilepsy or other seizure disorders.
Drug Interactions
Warfarin
Enhanced anticoagulant effect, leading to increased bleeding risk.
Theophylline
Increased plasma levels of theophylline.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Increased risk of CNS stimulation and convulsive seizures.
Antacids (aluminum, magnesium, calcium) and Iron Supplements
Reduced absorption of Trevox.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, dizziness, and confusion. Management should be symptomatic and supportive. Gastric lavage and activated charcoal may be considered. Hemodialysis is not effective in removing trevofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Trevox should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk; therefore, caution should be exercised when Trevox is administered to a nursing woman.

Clinical Trials
Clinical trials have demonstrated Trevox's efficacy and safety in various bacterial infections. Ongoing post-marketing surveillance and studies continue to monitor its long-term effects and resistance patterns.
Lab Monitoring
- Renal function tests (creatinine clearance) in patients with impaired renal function.
- Liver function tests (ALT, AST) if signs of hepatotoxicity develop.
- Monitoring of blood glucose levels in diabetic patients due to potential dysglycemia.
Doctor Notes
- Counsel patients on the black box warnings, particularly regarding tendinopathy, peripheral neuropathy, and CNS effects. Advise immediate discontinuation if these symptoms occur.
- Monitor renal function, especially in elderly or renally impaired patients, and adjust dose accordingly.
- Advise patients to avoid concurrent administration with divalent/trivalent cations (e.g., antacids, iron supplements) to prevent reduced absorption.
Patient Guidelines
- Complete the full course of medication, even if symptoms improve.
- Stay well-hydrated during treatment.
- Avoid exposure to excessive sunlight or artificial UV light.
- Report any new or worsening symptoms, especially tendon pain, numbness, or tingling.
