Tricef 200 mg Capsule contains Cefixime, a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, and middle ear. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg once daily or 100 mg twice daily. The dose can be increased to 400 mg once daily or 200 mg twice daily for severe infections. Treatment duration depends on the infection type, typically 7-14 days.
Elderly
No specific dose adjustment is required solely based on age for the elderly, unless there is significant renal impairment. Renal function should be assessed.
Renal_impairment
For creatinine clearance >60 mL/min, no adjustment. For 21-60 mL/min, 75% of the standard dose. For <20 mL/min or patients on continuous ambulatory peritoneal dialysis (CAPD), 50% of the standard dose.
How to Take
Administer orally, with or without food. Take the capsule whole with a glass of water. It is important to complete the full prescribed course of treatment.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to the disruption of bacterial cell wall integrity and eventual cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Primarily excreted by the kidneys via glomerular filtration and tubular secretion, with about 50% of an absorbed dose excreted unchanged in the urine within 24 hours.
Half life
Plasma elimination half-life is typically 3 to 4 hours.
Absorption
Approximately 40-50% orally absorbed, absorption is slightly delayed but not significantly affected by food.
Metabolism
Cefixime is not significantly metabolized in the liver; limited metabolism may occur.
Side Effects
Contraindications

Drug Interactions
Nifedipine
May increase the bioavailability of cefixime.
Carbamazepine
Elevated carbamazepine levels have been reported when co-administered with cefixime.
Oral contraceptives
No clinically significant interaction reported, but some cephalosporins may rarely reduce efficacy.
Warfarin and other anticoagulants
May increase prothrombin time and INR, potentially leading to bleeding. Close monitoring of coagulation parameters is recommended.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed after consulting a doctor. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
