Overview of the medicine
Tridol 100 mg Injection contains Tramadol Hydrochloride, a synthetic opioid analgesic. It is used for the management of moderate to moderately severe pain.
Uses & Indications
Dosage
Adults
Initial dose 50-100 mg IV or IM slowly every 4-6 hours as needed. Maximum daily dose usually 400 mg.
Elderly
For patients over 75 years, maximum daily dose should not exceed 300 mg.
Renal_impairment
Reduce total daily dose and/or extend dosing interval (e.g., 50-100 mg every 12 hours) for creatinine clearance <30 mL/min.
How to Take
Tridol 100 mg Injection should be administered slowly by intravenous infusion (over 2-3 minutes) or as a deep intramuscular injection. For IV infusion, it may be diluted with suitable solutions.
Mechanism of Action
Tramadol works by binding to mu-opioid receptors in the central nervous system. It also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effect.

IV: 5-10 minutes; IM: 15-30 minutes.
Excretion
Primarily excreted by the kidneys (approximately 80% as metabolites and unchanged drug).

Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration. Bioavailability is approximately 100% after IV administration.
Metabolism
Extensively metabolized in the liver by CYP2D6 and CYP3A4 enzymes to O-desmethyltramadol (active) and other inactive metabolites.

Contraindications
- Hypersensitivity to tramadol or any component of the injection
- Acute intoxication with alcohol, hypnotics, opioid analgesics, or psychotropic drugs
- Epilepsy not controlled by treatment
- Concomitant use with MAO inhibitors or within 14 days of stopping them
Drug Interactions
Warfarin
Increased INR and risk of bleeding.
Carbamazepine
Significantly reduces tramadol plasma levels and analgesic effect.
MAO Inhibitors
Risk of serotonin syndrome and seizures.
SSRIs, SNRIs, TCAs
Increased risk of serotonin syndrome and seizures.
CNS Depressants (e.g., benzodiazepines, other opioids, alcohol)
Increased risk of profound sedation, respiratory depression, coma, and death.

Symptoms of overdose include respiratory depression, somnolence, coma, seizures, miosis, cardiovascular collapse, and death. Management includes maintaining a patent airway, providing ventilatory support, and administering naloxone to reverse respiratory depression.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy, especially in the third trimester, due to risk of neonatal opioid withdrawal syndrome. Excreted into breast milk; not recommended during lactation.
