Description
Overview of the medicine
Trigent (Trimetazidine) is a cytoprotective anti-ischemic agent used for the symptomatic treatment of stable angina pectoris. It helps to improve myocardial energy metabolism and reduces the frequency of angina attacks.
Uses & Indications
Dosage
Adults
One 35 mg MR tablet twice daily (morning and evening) during meals.
Elderly
Same as adult dose, but caution is advised in patients with moderate renal impairment. Dose adjustment may be needed.
Renal_impairment
In moderate renal impairment (creatinine clearance 30-60 mL/min), one 35 mg MR tablet daily in the morning.
How to Take
Oral administration. Tablets should be taken with meals, swallowed whole with water, not chewed or crushed.
Mechanism of Action
Trimetazidine inhibits beta-oxidation of fatty acids by blocking long-chain 3-ketoacyl CoA thiolase, which shifts myocardial energy production from fatty acid oxidation to glucose oxidation. This helps to maintain cellular energy levels during ischemia.
Pharmacokinetics
Onset
Within a few hours for acute effects, chronic benefits accrue over days to weeks.
Excretion
Mainly through urine as unchanged drug.
Half life
Approximately 4-6 hours (for immediate release), 7-10 hours (for modified release).
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached within 2 hours.
Metabolism
Not extensively metabolized, primarily excreted unchanged.

Contraindications
- Hypersensitivity to Trimetazidine or any excipients
- Parkinson's disease, parkinsonian symptoms, tremors, restless leg syndrome and other related movement disorders
- Severe renal impairment (creatinine clearance < 30 mL/min)
