Description
Overview of the medicine
Ceftriaxone is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections, including serious ones.
Uses & Indications
Dosage
Adults
Typically 1-2 grams IV/IM once daily or in two divided doses. For 500mg strength, 500mg to 1g daily for milder infections. Max 4 grams/day.
Elderly
Same as adults, but caution should be exercised in patients with significant renal or hepatic impairment.
Renal_impairment
Dosage adjustment may be necessary in severe renal impairment, especially with concomitant hepatic impairment.
How to Take
Administered by intramuscular (IM) or intravenous (IV) injection. IV administration should be slow (over 2-4 minutes).
Mechanism of Action
Ceftriaxone works by inhibiting the bacterial cell wall synthesis, leading to bacterial cell death.

Rapid onset of action after intravenous administration.
Excretion
Excreted via both renal (kidney) and biliary (liver/bile) routes.

Approximately 6 to 9 hours.

Primarily not metabolized; small amount undergoes hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone, any other cephalosporin, or penicillins
- Neonates with hyperbilirubinemia, especially premature neonates
Drug Interactions
Loop diuretics
Increased risk of nephrotoxicity.
Aminoglycosides
May have synergistic effects, but renal function should be monitored.
Oral anticoagulants
May increase the risk of bleeding.
Calcium-containing solutions
Concomitant use with calcium-containing solutions is contraindicated in neonates and not recommended in other age groups.
Storage
Store below 30°C. Protect from light and moisture. The reconstituted solution has limited stability and should be used promptly.
Overdose
In case of overdose, symptomatic and supportive treatment should be given. Hemodialysis and peritoneal dialysis are not effective for removing ceftriaxone.
Pregnancy & Lactation
Pregnancy Category B. Excreted in breast milk in small amounts; use with caution in lactating mothers.
