Overview of the medicine
Montelukast is an orally active compound that inhibits leukotriene receptors, reducing inflammation and bronchoconstriction in asthma and allergic rhinitis.
Uses & Indications
Dosage
Adults
Montelukast 4 mg oral granules is typically prescribed for children aged 2-5 years. For adults and adolescents (15 years and older), the recommended dose of Montelukast is 10 mg once daily (usually a tablet form).
Elderly
No dosage adjustment generally required for Montelukast in elderly patients.
Renal_impairment
No dosage adjustment required for Montelukast in patients with renal impairment.
How to Take
Granules can be administered directly into the mouth, dissolved in a spoonful of cold or room temperature soft food (e.g., applesauce, ice cream, rice), or dissolved in breast milk or infant formula. Should be taken in the evening for asthma/rhinitis, or at least 2 hours before exercise for EIB.
Mechanism of Action
Selectively antagonizes cysteinyl leukotriene type 1 (CysLT1) receptors, preventing the actions of leukotrienes (LTC4, LTD4, LTE4), which are potent inflammatory mediators released from cells like mast cells and eosinophils.

Onset
Clinical improvement within one day for asthma.
Excretion
Primarily excreted in feces (about 86%) via bile; <0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration reached in about 2-4 hours for granules.
Metabolism
Extensively metabolized in the liver by CYP2C8, CYP3A4, and CYP2C9.

Contraindications
- Hypersensitivity to montelukast or any component of the formulation.
- Not for use in acute asthma attacks (not a rescue medication).
Drug Interactions
Phenobarbital, Rifampin
May decrease montelukast plasma levels.
Gemfibrozil, Itraconazole
May increase montelukast plasma levels, but usually not clinically significant.
