Description
Overview of the medicine
Trimotil (Trimetazidine) is a cardiac metabolic agent used to treat angina pectoris. It works by optimizing myocardial energy metabolism during ischemia.
Uses & Indications
Dosage
Adults
35 mg modified-release tablet twice daily (morning and evening) with meals.
Elderly
No specific dose adjustment is required, but caution is advised in patients with moderate renal impairment.
Renal_impairment
For moderate renal impairment (creatinine clearance 30-60 mL/min), the recommended dose is 35 mg once daily in the morning with breakfast.
How to Take
Swallow the tablet whole with water during meals. Do not chew or crush.
Mechanism of Action
Trimetazidine inhibits the 3-ketoacyl-CoA thiolase enzyme, leading to a partial inhibition of fatty acid oxidation. This shifts myocardial energy metabolism towards glucose oxidation, which is more efficient in ischemic conditions, thereby preserving cellular ATP levels.
Pharmacokinetics
Onset
Clinical effects may be observed within days to weeks.
Excretion
Primarily eliminated unchanged in urine. Renal clearance is significant.
Half life
Approximately 6 hours for conventional forms, prolonged for modified-release formulations (around 7-12 hours).
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration reached within 2-3 hours for modified-release formulations.
Metabolism
Minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Trimetazidine or any excipients
- Parkinson's disease, parkinsonian symptoms, tremors, restless leg syndrome, and other related movement disorders
- Severe renal impairment (creatinine clearance < 30 mL/min)
Drug Interactions
Levodopa
May exacerbate parkinsonian symptoms due to Trimetazidine's dopaminergic antagonist effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited data on overdose. Symptoms may include hypotension and flushing. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Not recommended during pregnancy due to lack of sufficient data. Use with caution in lactating mothers; consult a doctor.

Clinical Trials
Multiple clinical trials have demonstrated the efficacy of Trimetazidine in improving exercise capacity and reducing angina attacks in patients with stable angina.
Lab Monitoring
- No specific routine lab monitoring required for Trimetazidine. Monitor renal function in patients with renal impairment.
- Monitor for worsening parkinsonian symptoms.
Doctor Notes
- Trimetazidine is an add-on therapy for angina, not a substitute for standard anti-anginal treatment.
- Careful assessment of renal function is necessary before initiating treatment, especially in elderly patients.
- Inform patients about the risk of parkinsonian symptoms and advise them to report any such symptoms immediately.
Patient Guidelines
- Do not stop taking the medicine abruptly without consulting your doctor.
- Inform your doctor if you experience any unusual symptoms, especially movement disorders.
- This medicine is not for the acute treatment of angina attacks; use sublingual nitrates for acute attacks.
- Take with food to minimize stomach upset.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose.
Driving Precautions
Trimetazidine may cause dizziness and gait disturbances, which could affect the ability to drive or operate machinery. Patients should be warned of this risk.
Lifestyle Advice
- Maintain a healthy lifestyle, including a balanced diet and regular exercise, to support heart health.
- Avoid smoking and limit alcohol intake.
- Manage stress effectively.
