Description
Overview of the medicine
Tropidil (Tropicamide) is an anticholinergic agent used as a mydriatic (to dilate pupils) and cycloplegic (to paralyze accommodation) for diagnostic purposes during eye examinations and sometimes pre-operatively.
Uses & Indications
Dosage
Adults
For refraction, instill 1-2 drops of 1% solution; repeat after 5 minutes if necessary. For fundus examination, instill 1-2 drops of 0.5% solution 15-20 minutes prior to examination.
Elderly
Use with caution, as elderly patients may be more susceptible to systemic anticholinergic effects. Lower concentrations may be preferred.
Renal_impairment
No specific dosage adjustment recommended for ophthalmic use.
How to Take
For ophthalmic use only. Instill drops into the conjunctival sac. After instillation, press the lacrimal sac for one minute to minimize systemic absorption.
Mechanism of Action
Tropicamide blocks the responses of the sphincter pupillae muscle and the ciliary body to cholinergic stimulation, thereby causing mydriasis and cycloplegia. It competitively blocks muscarinic acetylcholine receptors.
Pharmacokinetics
Onset
Mydriasis within 15-20 minutes, cycloplegia within 20-45 minutes.
Excretion
Renal excretion if systemic absorption takes place.
Half life
Not well-defined for ophthalmic use; effects last 4-6 hours.
Absorption
Systemic absorption can occur, particularly in children and the elderly, but the primary action is local.
Metabolism
Limited information available; likely hepatic if significant systemic absorption occurs.
Side Effects
Contraindications
- Primary angle-closure glaucoma or a predisposition to it
- Hypersensitivity to tropicamide or any component of the formulation
Drug Interactions
Phenylephrine
May potentiate the mydriatic effect.
Other Anticholinergics
Additive anticholinergic effects may occur.
Storage
Store at room temperature (15-30°C). Protect from light. Do not freeze.
Overdose
Systemic overdose is rare with ophthalmic administration but can cause dry mouth, flushing, tachycardia, hyperpyrexia, blurred vision, and CNS symptoms like confusion, hallucinations. Treatment is symptomatic and supportive. Physostigmine may be used in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Exercise caution when administering to a nursing mother. Systemic absorption is minimal but possible.

Clinical Trials
Tropicamide is a well-established drug, and most clinical trials involve comparisons with other mydriatics or new formulations rather than primary efficacy trials.
Doctor Notes
- Caution in patients with a history of sensitivity to anticholinergic agents.
- Systemic absorption can lead to CNS effects (especially in children and the elderly) and cardiovascular effects.
- Always check for a narrow anterior chamber angle before instillation to avoid precipitating angle-closure glaucoma.
Patient Guidelines
- Do not drive or operate machinery until vision clears (which may take several hours).
- Protect eyes from bright light by wearing sunglasses.
- Avoid touching the dropper tip to prevent contamination.
Missed Dose Advice
Since Tropidil is primarily used for diagnostic purposes, a missed dose is not critical. Administer as soon as remembered, or proceed with the scheduled examination if the missed dose does not compromise the procedure.
Driving Precautions
Avoid driving or operating heavy machinery until vision returns to normal, as Tropidil can cause blurred vision and photophobia.
