Description
Overview of the medicine
Tropidil Plus is an ophthalmic solution containing Tropicamide and Phenylephrine Hydrochloride, used to dilate the pupil (mydriasis) and paralyze the ciliary muscle (cycloplegia) for diagnostic eye examinations and surgical procedures.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac, repeated in 5-10 minutes if necessary. For cycloplegia, repeat 2-3 times at 5-minute intervals.
Elderly
Use with caution due to increased risk of systemic side effects and acute angle-closure glaucoma. Lower doses may be appropriate.
Renal_impairment
No specific dose adjustment required for topical ophthalmic use.
How to Take
For ocular instillation only. Wash hands before use. Tilt head back, pull lower eyelid down to form a pouch, instill drops, and close eye gently. Apply pressure to the lacrimal sac for 1-2 minutes to minimize systemic absorption.
Mechanism of Action
Tropicamide acts as an anticholinergic, blocking muscarinic receptors in the iris sphincter and ciliary body, causing mydriasis and cycloplegia. Phenylephrine is an alpha-1 adrenergic agonist, stimulating the dilator muscle of the iris to cause mydriasis.
Pharmacokinetics
Onset
Mydriasis: 20-30 minutes. Cycloplegia: 20-45 minutes.
Excretion
Primarily via renal excretion if absorbed systemically.
Half life
Not well-defined for topical ocular use. Systemic half-life for Tropicamide is approximately 2-3 hours.
Absorption
Minimal systemic absorption from ocular surface, but can occur, especially in children or elderly. Peak systemic concentrations are low.
Metabolism
Tropicamide is metabolized by hydrolytic enzymes. Phenylephrine is metabolized by monoamine oxidase (MAO) and catechol-O-methyltransferase (COMT).
Side Effects
Contraindications
- •Known hypersensitivity to Tropicamide, Phenylephrine, or any component of the formulation.
- •Patients with narrow-angle glaucoma or a predisposition to angle-closure glaucoma (e.g., shallow anterior chamber).
- •Known or suspected pheochromocytoma (due to phenylephrine component).

Monoamine Oxidase Inhibitors (MAOIs): Concomitant use with phenylephrine can lead to hypertensive crisis.

Tricyclic Antidepressants (TCAs): May potentiate the pressor effects of phenylephrine.

Storage
Store in a cool, dry place below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose may lead to exaggerated systemic anticholinergic and adrenergic effects, including severe hypertension, tachycardia, headache, palpitation, dry mouth, blurred vision, flushing, and in rare cases, CNS manifestations like psychosis, convulsions, or coma. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother, as systemic absorption may occur.
