Description
Overview of the medicine
Trospi (Trospium chloride) is a muscarinic receptor antagonist used to treat overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency.
Uses & Indications
Dosage
Adults
20 mg twice daily, approximately one hour before meals, on an empty stomach.
Elderly
No specific dosage adjustment required, but caution advised.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), reduce dose to 20 mg once daily.
How to Take
Take orally, with water, on an empty stomach at least one hour before a meal or two hours after a meal. Do not crush or chew tablets.
Mechanism of Action
Trospi acts by competitively binding to muscarinic acetylcholine receptors, primarily M2 and M3, located on the detrusor muscle of the bladder. This blocks the effect of acetylcholine, leading to relaxation of the detrusor muscle and inhibition of involuntary bladder contractions, thereby increasing bladder capacity.
Pharmacokinetics
Onset
Clinical effects typically observed within 1-2 weeks.
Excretion
Primarily excreted unchanged via the kidneys.
Half life
Approximately 18 hours (for immediate-release formulation).
Absorption
Poorly absorbed from the gastrointestinal tract (bioavailability approx. 10%). Peak plasma concentrations reached within 4-6 hours.
Metabolism
Minimally metabolized; some ester hydrolysis and conjugation occur.
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Known hypersensitivity to Trospi (Trospium chloride) or any component of the formulation
Drug Interactions
Digoxin
May increase serum concentration of digoxin due to competition for renal tubular secretion.
Anticholinergic agents
Additive anticholinergic effects; avoid concomitant use.
Drugs that are actively secreted by the kidney (e.g., procainamide, vancomycin, metformin)
Potential for altered pharmacokinetics due to competition for renal tubular secretion.
Storage
Store at room temperature (below 30°C), protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms may include severe anticholinergic effects (e.g., blurred vision, dry mouth, urinary retention, tachycardia). Treatment is symptomatic and supportive; gastric lavage and activated charcoal may be considered. Physostigmine may be used as an antidote in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. It is not known whether Trospi (Trospium) is excreted in human milk; caution should be exercised when administered to a nursing woman.
