Description
Overview of the medicine
Trum-3 500 mg Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of serious bacterial infections in adults and children.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g every 8 hours by intravenous injection or infusion, depending on the severity and type of infection. Higher doses (up to 2 g every 8 hours) may be used for specific severe infections.
Elderly
No dosage adjustment is required in elderly patients with normal renal function. Dosage should be adjusted based on renal function if impaired.
Renal_impairment
Dosage reduction is necessary for patients with creatinine clearance <50 mL/min. Specific dose adjustments are guided by creatinine clearance levels.
How to Take
Trum-3 500 mg Injection should be administered intravenously by a healthcare professional. It can be given as a bolus injection or an infusion after reconstitution with appropriate sterile solvent (e.g., Water for Injection).
Mechanism of Action
Meropenem works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of cell wall integrity and ultimately bacterial cell death.
Pharmacokinetics
Onset
Rapid onset of action, typically within minutes of administration.
Excretion
Excreted primarily by the kidneys (approximately 70% as unchanged drug) within 12 hours.
Half life
Approximately 1 hour in individuals with normal renal function.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Primarily metabolized by hydrolysis to an inactive open beta-lactam ring metabolite; approximately 20% undergoes metabolic conversion.
Side Effects
Contraindications
- Known hypersensitivity to meropenem or any other carbapenem antibiotics
- Known severe hypersensitivity to any other type of beta-lactam antibacterial agents (e.g., penicillins or cephalosporins)
Drug Interactions
Probenecid
Concomitant administration of probenecid inhibits the renal tubular secretion of meropenem, resulting in increased meropenem plasma concentrations and a prolonged elimination half-life. Co-administration is not recommended.
Valproic acid/Sodium valproate
Meropenem may decrease valproic acid plasma concentrations to subtherapeutic levels, leading to loss of seizure control. Co-administration should be avoided.
Storage
Store unconstituted vials below 25°C (77°F). Protect from light and moisture. Keep out of reach of children. Reconstituted solution stability varies, use immediately or as per manufacturer guidelines.
Overdose
In the event of an overdose, symptomatic and supportive treatment should be initiated. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Meropenem is excreted in human milk, therefore caution should be exercised when administered to a nursing mother.

YesClinical Trials
Meropenem has undergone extensive clinical trials demonstrating its efficacy and safety in treating a wide range of severe bacterial infections across various patient populations, including pediatric and immunocompromised patients.
Lab Monitoring
- Renal function tests (serum creatinine, BUN) periodically, especially in patients with impaired renal function.
- Liver function tests (ALT, AST, alkaline phosphatase) if clinically indicated.
- Complete blood count (CBC) to monitor for hematological abnormalities.
Doctor Notes
- Consider Meropenem for empiric therapy in severe infections, especially in critically ill patients or those with resistant pathogens.
- Always obtain culture and sensitivity results when possible to guide definitive therapy and minimize resistance.
- Monitor for signs of CNS toxicity, especially in patients with pre-existing CNS disorders or renal impairment.
Patient Guidelines
- It is crucial to complete the entire prescribed course of Trum-3 Injection, even if your symptoms improve, to prevent the recurrence of infection and the development of antibiotic resistance.
- Inform your doctor immediately if you experience any signs of an allergic reaction (e.g., rash, itching, swelling of the face/throat, severe dizziness, trouble breathing).
- Report any new or worsening side effects, particularly severe diarrhea, to your healthcare provider.
Missed Dose Advice
If a dose is missed, it should be given as soon as possible. However, if it is almost time for the next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Some patients may experience dizziness or seizures. Patients should be advised to exercise caution when driving or operating machinery until they are sure Trum-3 does not adversely affect them.
Lifestyle Advice
- Maintain good hygiene to prevent further infections.
- Stay well-hydrated during the treatment period.
- Avoid alcohol consumption while taking this antibiotic.
