Description
Overview of the medicine
Tysinor 500 mg tablet contains Paracetamol, which is used for the relief of mild to moderate pain and to reduce fever.
Uses & Indications
Dosage
Adults
1-2 tablets (500-1000 mg) every 4-6 hours as needed. Do not exceed 8 tablets (4000 mg) in 24 hours.
Elderly
Similar to adult dosage, but caution should be exercised in patients with impaired hepatic or renal function.
Renal_impairment
Dosage interval may need to be extended. For severe renal impairment, consider extending the interval to 6-8 hours. Consult a doctor.
How to Take
Take the tablet orally with water. It can be taken with or without food. Do not crush or chew the tablet.
Mechanism of Action
Paracetamol is thought to act primarily by inhibiting prostaglandin synthesis in the central nervous system, which leads to its analgesic and antipyretic effects. It has weak anti-inflammatory effects.
Pharmacokinetics
Onset
Analgesic effect begins within 30-60 minutes; antipyretic effect within 30 minutes.
Excretion
Excreted mainly by the kidneys as glucuronide and sulfate conjugates (90-100% within 24 hours).
Half life
Plasma elimination half-life is 1-4 hours in individuals with normal liver function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation (45-55%) and sulfation (20-30%). A small amount is metabolized by cytochrome P450 to a highly reactive intermediate (NAPQI), which is detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any other component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Chronic alcohol consumption increases the risk of hepatotoxicity.
Lamotrigine
Paracetamol may reduce the half-life of lamotrigine.
Cholestyramine
Reduces paracetamol absorption if taken within one hour of paracetamol.
Warfarin and other coumarins
May increase the anticoagulant effect and risk of bleeding with prolonged regular daily use, requiring INR monitoring.
Metoclopramide and Domperidone
Increase the rate of paracetamol absorption.
Isoniazid, Phenytoin, Carbamazepine, Barbiturates
May increase the risk of paracetamol hepatotoxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose of paracetamol can cause severe, potentially fatal liver damage (hepatotoxicity). Symptoms may not appear for up to 24-48 hours and include nausea, vomiting, abdominal pain, and sweating. In case of suspected overdose, seek immediate medical attention. Treatment involves activated charcoal (if administered early) and N-acetylcysteine.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation when used at recommended doses for the shortest possible duration. However, pregnant or breastfeeding women should consult a doctor before use.
