Ciprofloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
For most infections, 500 mg orally twice daily. Dosage may vary from 250 mg to 750 mg twice daily depending on the infection type and severity.
Elderly
Dosage adjustment may be needed based on renal function. Monitor renal function closely.
Renal_impairment
Dose reduction is required for creatinine clearance < 50 mL/min. For severe impairment (CrCl < 30 mL/min), reduce dose by 50% or extend dosing interval.
How to Take
Take the capsule whole with a glass of water, preferably on an empty stomach (2 hours before or 2 hours after a meal). Do not take with dairy products (milk, yogurt) or calcium-fortified juices. Avoid taking with antacids containing magnesium or aluminum.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Rapid, typically within 1-2 hours of administration.
Excretion
Primarily via kidneys (renal excretion of unchanged drug and metabolites); some non-renal clearance.

Absorption

Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- •Concomitant administration with Tizanidine.
- •History of tendon disorders related to fluoroquinolone administration.
Drug Interactions
Warfarin
Enhanced anticoagulant effect of Warfarin, increasing bleeding risk. Monitor INR.
Tizanidine
Concomitant use is contraindicated due to increased plasma concentrations of Tizanidine, leading to hypotension and sedation.
Theophylline
Ciprofloxacin can increase plasma concentrations of Theophylline, leading to increased risk of toxicity (e.g., seizures). Monitor Theophylline levels.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Antacids (Magnesium/Aluminum), Sucralfate, Didanosine (chewable/buffered tablets)
Reduced absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from moisture and direct light. Keep out of reach of children. Do not freeze.
Overdose
In case of acute overdose, gastric lavage may be considered. Supportive treatment and hydration are crucial. Hemodialysis or peritoneal dialysis remove only a small amount of ciprofloxacin.
Pregnancy & Lactation
Generally not recommended during pregnancy due to potential risks to the fetus (e.g., arthropathy observed in immature animals). Use only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk; breastfeeding is generally not recommended during treatment.
