Description
Overview of the medicine
Cefepime is a broad-spectrum, fourth-generation cephalosporin antibiotic used to treat various bacterial infections, including complicated and uncomplicated urinary tract infections, pneumonia, skin and skin structure infections, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
1-2 gm IV/IM every 8-12 hours, depending on infection severity. For febrile neutropenia, 2 gm IV every 8 hours.
Elderly
Dosage adjustment required based on renal function, similar to adults with renal impairment.
Renal_impairment
Dosage must be adjusted based on creatinine clearance (e.g., 1 gm every 24 hours for CrCl 30-60 mL/min, 0.5 gm every 24 hours for CrCl 11-29 mL/min).
How to Take
Intravenous (IV) infusion over 30 minutes or Intramuscular (IM) deep injection. Reconstitute powder with appropriate diluent (e.g., Sterile Water for Injection, 0.9% Sodium Chloride).
Mechanism of Action
Cefepime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), particularly PBP-3, which results in the inhibition of peptidoglycan synthesis, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily renal excretion (85% as unchanged drug in urine).
Half life
Approximately 2 hours in adults with normal renal function.
Absorption
Administered intravenously, providing 100% bioavailability.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefepime, other cephalosporins, penicillins, or other beta-lactam antibiotics.
- Patients with a history of severe immediate hypersensitivity reactions to penicillin.
Drug Interactions
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity.
Loop Diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity when co-administered with aminoglycosides.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect.
Storage
Store powder at controlled room temperature (20-25°C), protected from light. Reconstituted solution stable for 24 hours at room temperature or 7 days under refrigeration (2-8°C).
Overdose
Symptoms may include encephalopathy, seizures, and neuromuscular excitability. Management involves supportive care and hemodialysis to remove the drug.
Pregnancy & Lactation
Use during pregnancy only if clearly needed after careful risk-benefit assessment. Excreted in breast milk in low concentrations; use with caution in breastfeeding mothers.

Doctor Notes
- Consider renal function adjustment in elderly and renally impaired patients.
- Monitor for neurological adverse events, especially in patients with renal impairment or those receiving high doses.
- Culture and sensitivity testing recommended before initiating treatment.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Report any severe diarrhea, rash, or allergic reactions immediately.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered, unless it is almost time for the next scheduled dose. Do not double the dose to catch up.
