Description
Overview of the medicine
Cefepime is a fourth-generation cephalosporin antibiotic used to treat various moderate to severe bacterial infections, including pneumonia, urinary tract infections, skin and soft tissue infections, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
Typical dose: 1-2 g IV/IM every 8-12 hours, depending on the severity and type of infection. Max 2 g every 8 hours.
Elderly
Dosage adjustment is necessary based on renal function. Generally, lower doses or extended intervals are recommended.
Renal_impairment
Significant dosage reduction is required based on creatinine clearance. Consult a specific dosing table for guidance.
How to Take
Ultrapime (Cefepime) is administered by intravenous (IV) infusion over 30 minutes or by deep intramuscular (IM) injection. It must be reconstituted with an appropriate sterile diluent (e.g., Sterile Water for Injection, 0.9% Sodium Chloride Injection).
Mechanism of Action
Cefepime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs).
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily renal excretion, mainly through glomerular filtration.
Half life
Approximately 2 hours in adults with normal renal function.
Absorption
Rapidly and completely absorbed after intravenous (IV) or intramuscular (IM) administration. Peak plasma concentrations are achieved shortly after IV infusion.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefepime or any other cephalosporin
- History of severe hypersensitivity reaction to any other type of beta-lactam antibacterial agents (e.g., penicillins, monobactams, carbapenems)
Drug Interactions
Loop Diuretics
May increase cefepime concentrations and toxicity.
Aminoglycosides
Increased risk of nephrotoxicity and ototoxicity when co-administered with cefepime.
Oral Anticoagulants
May increase the anticoagulant effect (monitor INR).
Storage
Store dry powder vials below 25°C (77°F) and protect from light. Reconstituted solutions usually stable for 24 hours at room temperature or 7 days under refrigeration (2-8°C). Refer to product information for specific stability data.
Overdose
In case of overdose, symptomatic treatment should be initiated. Hemodialysis may be useful in aiding removal of cefepime from the body, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, cefepime should be used during pregnancy only if clearly needed. Cefepime is excreted in breast milk in small quantities; caution should be exercised when administered to a nursing mother. Consult a physician.

YesClinical Trials
Cefepime has undergone extensive clinical trials demonstrating its efficacy and safety in treating a wide range of bacterial infections, including those caused by multi-drug resistant organisms. It has been compared with other broad-spectrum antibiotics and shown to be non-inferior or superior in various settings.
Lab Monitoring
- Monitor renal function (creatinine, BUN) regularly, especially in patients with pre-existing renal impairment or during high-dose therapy.
- Complete Blood Count (CBC) with differential.
- Liver Function Tests (LFTs) periodically.
Doctor Notes
- Crucial agent for empiric therapy in severe hospital-acquired and healthcare-associated infections, especially where Pseudomonas aeruginosa is suspected.
- Careful dose adjustment is paramount in patients with renal impairment to prevent neurotoxicity and other adverse effects.
- Consider combination therapy with anaerobes coverage for intra-abdominal infections.
Patient Guidelines
- Complete the entire prescribed course of Ultrapime, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Report any severe or persistent side effects, especially severe diarrhea or neurological changes, to your doctor immediately.
- Inform your healthcare provider about all medications, supplements, and herbal products you are taking.
Missed Dose Advice
If a dose is missed, administer it as soon as you remember, unless it is almost time for the next scheduled dose. Do not double the dose to catch up. Contact your doctor or nurse if unsure.
Driving Precautions
Cefepime can rarely cause dizziness or other neurological disturbances (e.g., encephalopathy, seizures), particularly in patients with renal impairment who receive unadjusted doses. Exercise caution when driving or operating machinery.
Lifestyle Advice
- Maintain adequate hydration during treatment.
- Avoid consuming alcohol if it exacerbates gastrointestinal side effects.
