Ultrasef 125 mg Suspension contains Cefuroxime Axetil, which is a second-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used in adults at 125 mg suspension strength; higher strengths or tablet forms are prescribed for adults. (e.g., 250 mg or 500 mg tablets twice daily).
Elderly
Generally, no dose adjustment is needed unless severe renal impairment is present. Follow pediatric dosing guidelines if used for specific indications.
Renal_impairment
Dosage interval should be adjusted based on creatinine clearance. For CrCl 10-29 mL/min, administer standard dose every 24 hours. For CrCl <10 mL/min, administer standard dose every 48 hours.
How to Take
For oral administration. Shake the bottle well before each use. It is recommended to take Ultrasef Suspension with food to enhance absorption. Use the provided measuring device for accurate dosing.
Mechanism of Action
Cefuroxime Axetil acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of peptidoglycan synthesis, causing bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved approximately 2 to 3 hours after oral administration with food.
Excretion
Excreted unchanged by glomerular filtration and tubular secretion in the urine. Approximately 50% of the dose is excreted within 12 hours.
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract and rapidly hydrolyzed by non-specific esterases in the intestinal mucosa and blood to cefuroxime. Absorption is enhanced when taken with food.
Metabolism
Cefuroxime itself is not metabolized; the axetil ester is hydrolyzed to the active drug cefuroxime.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, any other cephalosporin antibiotics, or any of the excipients of Ultrasef Suspension.
- History of severe hypersensitivity (e.g., anaphylactic reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral Anticoagulants
Cefuroxime may affect gut flora, leading to lower vitamin K levels and potentiation of anticoagulant effects. Monitor INR/PT.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise using additional barrier contraception.
Antacids/H2 blockers/PPIs
May decrease the bioavailability of cefuroxime axetil, especially if taken shortly after meals. Administer at least an hour before or after these agents.
Storage
Store the dry powder at room temperature (below 30°C) in a dry place, away from direct sunlight and moisture. Once reconstituted, store the suspension in a refrigerator (2°C-8°C) and do not freeze.

Symptoms of overdose include convulsions and other neurological disorders. Management is supportive; hemodialysis or peritoneal dialysis can remove cefuroxime from the body.
