Description
Overview of the medicine
Umactin BD contains Ursodeoxycholic Acid, a bile acid used to dissolve cholesterol gallstones and treat certain liver diseases like Primary Biliary Cholangitis (PBC).
Uses & Indications
Dosage
Adults
For gallstone dissolution: 8-10 mg/kg/day, usually a single dose at bedtime or divided into 2-3 doses. For PBC: 13-15 mg/kg/day, usually divided into 2-4 doses.
Elderly
No specific dosage adjustment needed, but monitor liver function.
Renal_impairment
No dosage adjustment necessary as it's primarily excreted in bile/feces.
How to Take
Take with food or milk to optimize absorption. For gallstone dissolution, a single dose at bedtime is often recommended.
Mechanism of Action
Ursodeoxycholic Acid reduces the cholesterol content of bile, leading to the dissolution of cholesterol gallstones. It also protects liver cells from toxic bile acids and has anti-inflammatory properties.
Pharmacokinetics
Onset
Gallstone dissolution takes months; PBC symptom improvement can be slower.
Excretion
Primarily excreted in feces.
Half life
3.5-5.8 days (after continuous therapy).
Absorption
Well absorbed after oral administration, undergoes enterohepatic recirculation.
Metabolism
Metabolized in the liver to various bile acids.
Side Effects
Contraindications
- Hypersensitivity to bile acids
- Acute cholecystitis
- Calcified cholesterol gallstones
- Non-functioning gallbladders
- Biliary obstruction
Drug Interactions
Antacids containing aluminum
May reduce UDCA absorption.
Cholestyramine, Colestipol, Colesevelam
May reduce UDCA absorption; take UDCA at least 2 hours before or 4 hours after.
Oral contraceptives, Estrogen, Clofibrate
May increase cholesterol saturation of bile, counteracting UDCA's effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are typically diarrhea. Treatment involves symptomatic and supportive measures.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Excretion into breast milk is minimal, likely safe for nursing infants.
