Unicontin 200 mg is an extended-release tablet containing Theophylline, used to prevent and treat symptoms of asthma and other lung diseases like COPD (Chronic Obstructive Pulmonary Disease) and chronic bronchitis. It helps to relax the muscles in the airways, making breathing easier.
Uses & Indications
Dosage
Adults
Usually 200 mg twice daily, adjusted based on clinical response and serum theophylline levels. Max 400 mg twice daily.
Elderly
Lower doses may be required due to decreased clearance; monitor closely.
Renal_impairment
No specific dose adjustment for renal impairment, but caution advised due to increased risk of toxicity from metabolites.
How to Take
Take the tablet orally, whole, with a glass of water, with or without food. Do not chew, crush, or break the extended-release tablet, as this can lead to rapid absorption and potential toxicity.
Mechanism of Action
Theophylline is a non-selective phosphodiesterase inhibitor and an adenosine receptor antagonist. By inhibiting phosphodiesterase, it increases intracellular cyclic AMP (cAMP) levels, which leads to bronchodilation, smooth muscle relaxation, and inhibition of inflammatory mediator release. As an adenosine receptor antagonist, it also contributes to bronchodilation and modulates inflammation.

Onset
Gradual onset over several hours due to extended-release formulation.
Excretion
Mainly excreted by the kidneys (approximately 10% as unchanged drug, the rest as metabolites).

Highly variable, typically 5-10 hours in healthy adults, shorter in smokers and children, longer in elderly and those with liver impairment.

Contraindications

Drug Interactions
Lithium
Theophylline may increase lithium excretion, decreasing its levels.
Beta-blockers

May decrease theophylline levels by inducing its metabolism, reducing efficacy.
Cimetidine, Erythromycin, Ciprofloxacin, Fluvoxamine, Oral Contraceptives
May increase theophylline levels by inhibiting its metabolism, leading to toxicity.
Storage
Store below 30°C, in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, persistent headache, restlessness, insomnia, tremor, palpitations, and in severe cases, cardiac arrhythmias and seizures. Management involves discontinuing the drug, activated charcoal to reduce absorption, and supportive care. Hemodialysis or hemoperfusion may be considered for severe or refractory toxicity.
Pregnancy & Lactation
Pregnancy Category C. Theophylline crosses the placenta and is excreted in breast milk. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Monitor breastfed infants for irritability or other signs of toxicity.
