Description
Overview of the medicine
Urinide 5 mg Tablet contains Furosemide, a potent loop diuretic used to treat edema (fluid retention) associated with various conditions and also for the management of hypertension.
Uses & Indications
Dosage
Adults
For edema: Initial 20-80 mg orally as a single dose, adjusted according to response. For 5 mg tablet, dose might be 1-2 tablets daily for very mild cases or in combination therapy, as directed by physician. For hypertension: Initial 20-40 mg twice daily, adjusted.
Elderly
Lower initial doses (e.g., 20 mg once daily) may be required due to increased sensitivity and potential for renal impairment. For 5 mg, careful monitoring is essential.
Renal_impairment
Dosage adjustment may be necessary. In severe renal failure, higher doses might be needed to achieve diuresis, but careful monitoring of fluid and electrolyte balance is crucial.
How to Take
Take orally, with or without food. It is often recommended to take it in the morning to avoid nocturnal diuresis. Follow your doctor's instructions precisely.
Mechanism of Action
Furosemide acts on the ascending limb of the loop of Henle in the kidney, inhibiting the reabsorption of sodium and chloride, leading to increased excretion of water, sodium, chloride, and potassium.
Pharmacokinetics
Onset
Oral: 30-60 minutes; IV: 5 minutes.
Excretion
Primarily excreted unchanged in urine via tubular secretion; some also in feces.
Half life
Approximately 1-1.5 hours in healthy individuals, may be prolonged in renal or hepatic impairment.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is variable (40-70%).
Metabolism
Partially metabolized in the liver, forming glucuronide conjugates.
Side Effects
Contraindications
- •Anuria (absence of urine production)
- •Hepatic coma or precoma
- •Severe electrolyte depletion (e.g., hypokalemia, hyponatremia)
- •Hypersensitivity to furosemide or sulfonamides
Drug Interactions
Digoxin
Increased risk of digitalis toxicity due to hypokalemia.
Lithium
Increased serum lithium levels, leading to toxicity.
Aminoglycoside Antibiotics
Increased risk of ototoxicity (hearing damage).
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce diuretic and antihypertensive effects of furosemide.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dehydration, electrolyte imbalance (e.g., hypokalemia, hyponatremia), hypovolemia, and hypotension. Management involves symptomatic and supportive care, including fluid and electrolyte replacement.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Furosemide is excreted in breast milk; therefore, exercise caution or avoid use during lactation.
