Description
Overview of the medicine
Urinom (Tamsulosin Hydrochloride) is an alpha-blocker medication primarily used to treat the symptoms of benign prostatic hyperplasia (BPH), such as difficulty urinating, weak stream, and frequent urination. It works by relaxing the muscles in the prostate and bladder neck, making it easier to urinate.
Uses & Indications
Dosage
Adults
0.4 mg orally once daily, approximately 30 minutes after the same meal each day. May be increased to 0.8 mg orally once daily after 2-4 weeks if necessary.
Elderly
No dosage adjustment is usually required for elderly patients.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Take orally once daily, approximately 30 minutes after the same meal each day. Swallow the capsule whole; do not crush, chew, or open it.
Mechanism of Action
Tamsulosin selectively blocks alpha-1A and alpha-1D adrenoceptors located in the prostate, prostatic capsule, bladder neck, and prostatic urethra. This blockade causes relaxation of the smooth muscle in these tissues, thereby decreasing resistance to urine flow and improving BPH symptoms.
Pharmacokinetics
Onset
Improvement in symptoms within 1-2 weeks; full effect in 2-4 weeks.
Excretion
Primarily excreted in the urine, mostly as metabolites.
Half life
Approximately 9-15 hours in plasma.
Absorption
Well absorbed from the gastrointestinal tract, nearly 90% bioavailable when taken after food.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2D6.
Side Effects
Contraindications
- Hypersensitivity to Tamsulosin or any component of the formulation
- History of orthostatic hypotension
- Severe hepatic impairment
Drug Interactions
Warfarin
Monitor INR carefully, as Tamsulosin may displace warfarin from protein binding sites.
Other Alpha-Adrenergic Blocking Agents
Increased risk of hypotensive effects.
Strong CYP2D6 inhibitors (e.g., Paroxetine)
May increase Tamsulosin exposure. Use with caution.
PDE5 Inhibitors (e.g., Sildenafil, Tadalafil)
Potential for additive hypotensive effects. Administer with caution.
Strong CYP3A4 inhibitors (e.g., Ketoconazole)
May significantly increase Tamsulosin exposure. Avoid concomitant use.
Storage
Store below 30°C, in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose may lead to severe hypotensive effects, potentially requiring cardiovascular support. Treatment is symptomatic and supportive. Laying the patient supine and administering fluids may be helpful. Vasopressors may be used if needed.
Pregnancy & Lactation
Urinom is not indicated for use in women. No data on use in pregnancy or lactation.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of Tamsulosin in treating symptoms of BPH. These trials typically involve placebo-controlled, double-blind studies assessing improvements in urinary flow rates and symptom scores.
Lab Monitoring
- No specific routine lab monitoring is required for Tamsulosin. Blood pressure should be monitored, especially after initial dosing and dose titration.
Doctor Notes
- Assess patient for prostate cancer before initiating therapy and periodically thereafter, as BPH and prostate cancer may coexist.
- Inform patients about the risk of orthostatic hypotension and IFIS, especially before any eye surgery.
- Monitor blood pressure regularly, particularly during initial therapy and dose adjustments.
Patient Guidelines
- Take Urinom exactly as prescribed by your doctor.
- Swallow the capsule whole; do not crush, chew, or open it.
- Be aware of symptoms of orthostatic hypotension (dizziness, lightheadedness) when standing up, especially after the first dose.
- Inform your eye surgeon that you are taking Urinom if you are scheduled for cataract or glaucoma surgery.
Missed Dose Advice
If a dose is missed, take the next scheduled dose at the usual time. Do not take a double dose to make up for a missed one.
Driving Precautions
Urinom may cause dizziness or lightheadedness, especially when standing up quickly. Patients should be advised to use caution when driving or operating machinery until they know how Urinom affects them.
Lifestyle Advice
- Avoid sudden changes in position, especially from lying down or sitting to standing, to minimize dizziness.
- Maintain a healthy lifestyle, including regular exercise and a balanced diet, which may help manage BPH symptoms.
