Description
Overview of the medicine
Urobak 100 mg Tablet contains Nitrofurantoin, an antibacterial agent used to treat and prevent uncomplicated urinary tract infections (UTIs) caused by susceptible bacteria. It works by interfering with bacterial enzyme systems.
Uses & Indications
Dosage
Adults
For uncomplicated UTI: 50-100 mg four times daily for 7 days. For prophylaxis: 50-100 mg once daily at bedtime.
Elderly
Use with caution due to potential for reduced renal function; lower doses may be required.
Renal_impairment
Contraindicated in severe renal impairment (CrCl <60 mL/min) due to risk of toxicity and lack of efficacy. Avoid use if CrCl <60 mL/min.
How to Take
Take orally with food to enhance absorption and reduce gastrointestinal upset. Swallow tablets whole with a glass of water.
Mechanism of Action
Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that inactivate or alter bacterial ribosomal proteins and other macromolecules. This leads to inhibition of DNA, RNA, and protein synthesis, and cell wall formation, eventually causing bacterial cell death.

Absorption
Rapidly and completely absorbed from the GI tract. Food enhances absorption.

Approximately 50% metabolized in various body tissues, excluding the kidneys.
Side Effects
Contraindications
- •Severe renal impairment (CrCl <60 mL/min)
- •Known hypersensitivity to nitrofurantoin or any excipients
- •History of cholestatic jaundice/hepatic dysfunction associated with nitrofurantoin
- •Infants younger than one month of age (due to risk of hemolytic anemia)
- •Third trimester of pregnancy (due to risk of hemolytic anemia in the neonate)
Drug Interactions
Probenecid and Sulfinpyrazone
May decrease renal excretion of nitrofurantoin, leading to increased plasma levels and toxicity, and decreased urinary efficacy.
Magnesium trisilicate antacids
May reduce nitrofurantoin absorption.
Live attenuated typhoid vaccine
Antibiotics may reduce the therapeutic effect of the vaccine.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include gastrointestinal distress (nausea, vomiting), peripheral neuropathy. Management is primarily supportive care, hydration, and monitoring of vital signs. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Avoid during third trimester due to risk of hemolytic anemia in the neonate. Excreted in breast milk, use with caution in nursing mothers, contraindicated in infants <1 month old.
