Description
Overview of the medicine
Uroblock (Solifenacin Succinate) is an anticholinergic agent used to treat symptoms of overactive bladder, such as urinary urgency, frequency, and incontinence.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. May be increased to 10 mg once daily based on efficacy and tolerability.
Elderly
No dosage adjustment is required for elderly patients, but caution is advised.
Renal_impairment
For severe renal impairment (creatinine clearance <30 mL/min), do not exceed 5 mg once daily.
How to Take
Take orally with water, with or without food. Do not chew, crush, or break the tablet.
Mechanism of Action
Solifenacin is a competitive muscarinic receptor antagonist. By inhibiting muscarinic receptors, particularly M3 receptors in the bladder, it reduces involuntary detrusor muscle contractions, thereby decreasing urinary urgency, frequency, and incontinence.
Pharmacokinetics
Onset
Significant effects observed within 1 week, maximal effect within 4-8 weeks.
Excretion
Excreted via urine (approximately 69%) and feces (approximately 23%).
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract, with an absolute bioavailability of approximately 90%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4).
Side Effects
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Severe hepatic impairment
Drug Interactions
Other anticholinergic agents
May enhance anticholinergic effects.
Prokinetic agents (e.g., Metoclopramide)
May reduce their effect.
Strong CYP3A4 inhibitors (e.g., Ketoconazole)
May increase solifenacin exposure; limit dose to 5 mg.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe anticholinergic effects like dry mouth, blurred vision, urinary retention, and tachycardia. Treatment is symptomatic and supportive; consider gastric lavage and activated charcoal, and administer physostigmine in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is unknown if solifenacin is excreted in human milk.
