Overview of the medicine
Uromax (Tamsulosin Hydrochloride) is an alpha-blocker used to treat the signs and symptoms of benign prostatic hyperplasia (BPH) by relaxing the muscles in the prostate and bladder neck, making it easier to urinate.
Uses & Indications
Dosage
Adults
0.4 mg orally once daily, taken approximately 30 minutes after the same meal each day. May be increased to 0.8 mg once daily if needed, after several weeks.
Elderly
No specific dose adjustment is required.
Renal_impairment
No dose adjustment is necessary for patients with mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 10 mL/min) as it has not been studied.
How to Take
Take one capsule orally, approximately 30 minutes after the same meal each day. The capsule should be swallowed whole; do not crush, chew, or open it.
Mechanism of Action
Tamsulosin selectively blocks alpha-1A adrenergic receptors in the prostate, prostatic capsule, bladder neck, and prostatic urethra. Blocking these receptors causes relaxation of the smooth muscles, which improves urine flow and reduces symptoms of BPH.
Pharmacokinetics
Onset
Symptomatic improvement may be observed within 4-8 hours, but full effect takes several weeks.
Excretion
Primarily excreted in the urine (76%) as metabolites, with a smaller amount in feces (21%).
Half life
Approximately 9-15 hours (plasma half-life).
Absorption
Well absorbed orally, with bioavailability enhanced by food.
Metabolism
Extensively metabolized by hepatic CYP450 enzymes, primarily CYP3A4 and CYP2D6.

Drug Interactions
Warfarin
Monitor INR carefully as tamsulosin may rarely affect prothrombin time.
Other Alpha-Adrenergic Blocking Agents

PDE5 inhibitors (e.g., sildenafil, tadalafil)
Concurrent use may result in symptomatic hypotension; use with caution.
Potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May significantly increase tamsulosin exposure; use with caution.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to severe hypotensive effects, potentially requiring supportive measures such as lying patient down and administering intravenous fluids. Vasopressors may be used if necessary. Monitor renal function.
