Description
Overview of the medicine
Utracet is a combination medicine containing Tramadol Hydrochloride and Paracetamol. It is used for the management of moderate to moderately severe pain, often when other pain relievers are insufficient.
Uses & Indications
Dosage
Adults
Recommended initial dose is 1-2 tablets every 4-6 hours as needed for pain relief, not to exceed 8 tablets (300 mg Tramadol/2600 mg Paracetamol) per day.
Elderly
For patients over 75 years, maximum daily dose should not exceed 6 tablets due to risk of increased plasma concentrations.
Renal_impairment
In patients with creatinine clearance <30 mL/min, the dosing interval should be increased to not more than two tablets every 12 hours.
How to Take
Administer orally. Can be taken with or without food.
Mechanism of Action
Tramadol acts as a weak opioid agonist and inhibits the reuptake of norepinephrine and serotonin. Paracetamol inhibits prostaglandin synthesis primarily in the central nervous system, leading to analgesic and antipyretic effects. The combination provides synergistic pain relief.

Half life
Tramadol: approximately 5-7 hours; Paracetamol: approximately 2-3 hours.
Absorption
Both Tramadol and Paracetamol are rapidly and well absorbed after oral administration. Tramadol bioavailability is about 75%, Paracetamol 60-90%.
Metabolism
Both are primarily metabolized in the liver. Tramadol by CYP2D6 and CYP3A4 to O-desmethyltramadol (active) and N-desmethyltramadol. Paracetamol undergoes hepatic glucuronidation and sulfation, and also by CYP2E1.

Contraindications
- Hypersensitivity to Tramadol, Paracetamol, or opioids
- Acute intoxication with alcohol, hypnotics, analgesics, opioids, or psychotropic drugs
- Severe respiratory depression
- Concomitant use of MAO inhibitors or within 14 days of stopping them
- Severe hepatic insufficiency
- Uncontrolled epilepsy
Drug Interactions
Carbamazepine
Decreased Tramadol plasma concentrations.
MAO Inhibitors
Contraindicated due to risk of serotonin syndrome and seizures.
Serotonergic drugs (e.g., SSRIs, SNRIs)
Increased risk of serotonin syndrome.
Warfarin and other coumarin derivatives
Increased INR and bleeding risk.
CNS Depressants (e.g., benzodiazepines, alcohol)
Increased risk of respiratory depression, sedation, coma, and death.

Symptoms include respiratory depression, seizures, CNS depression, and severe hepatic damage (from paracetamol). Management involves supportive care, naloxone for opioid effects, and N-acetylcysteine for paracetamol overdose.

Pregnancy Category C. Not recommended during pregnancy unless the potential benefit outweighs the potential risk. Can cause neonatal opioid withdrawal syndrome. Excreted in breast milk, use with caution or avoid during lactation.
