Overview of the medicine
Valestra 1 mg Tablet contains Estradiol Valerate, an estrogen used for hormone replacement therapy (HRT) to relieve menopausal symptoms and prevent postmenopausal osteoporosis. It can also be used for female hypogonadism.
Uses & Indications
Dosage
Adults
For menopausal symptoms: Typically 1-2 mg daily, continuously or cyclically as prescribed by a physician. For osteoporosis prevention: 1 mg daily. For hypogonadism: 1-2 mg daily.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential increased sensitivity and comorbidities.
Renal_impairment
No specific dosage adjustment is usually necessary. Use with caution in severe renal impairment.
How to Take
Valestra tablets should be taken orally, with or without food, preferably at the same time each day. Swallow the tablet whole with water.
Mechanism of Action
Estradiol Valerate is a synthetic estrogen that is converted in the body to estradiol, the primary female sex hormone. It binds to and activates estrogen receptors in target tissues, mimicking the physiological effects of endogenous estrogen.

Excreted mainly in the urine as glucuronide and sulfate conjugates.
Half life

Rapidly and completely absorbed from the gastrointestinal tract, then rapidly hydrolyzed to estradiol and valeric acid.
Metabolism
Primarily in the liver by conjugation and hydroxylation, also undergoes significant first-pass metabolism.
Side Effects
Contraindications
- •Known, suspected, or history of breast cancer.
- •Known or suspected estrogen-dependent malignant tumors.
- •Undiagnosed vaginal bleeding.
- •Active or history of venous thromboembolism (DVT, PE).
- •Active or recent arterial thromboembolic disease (e.g., angina, myocardial infarction, stroke).
- •Acute liver disease or history of liver disease where liver function tests have not returned to normal.
- •Porphyria.
- •Hypersensitivity to the active substance or any excipients.
Drug Interactions
Anticoagulants (e.g., Warfarin)
Estrogens may affect coagulation factors, potentially altering the effects of anticoagulants.
Thyroid Hormone Replacement (e.g., Levothyroxine)
Estrogens may increase thyroid binding globulin, leading to increased thyroid hormone requirements.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase estrogen levels, potentially leading to increased side effects.

Storage

Overdose
Symptoms of overdose may include nausea, vomiting, breast tenderness, abdominal pain, drowsiness/fatigue, and withdrawal bleeding in women. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Valestra is contraindicated during pregnancy. If pregnancy occurs during treatment, discontinue immediately. It is not recommended during lactation as estrogens are excreted in breast milk and may reduce milk production.
