Description
Overview of the medicine
Valestra 2 mg tablet contains Estradiol, an estrogen hormone, primarily used for hormone replacement therapy (HRT) to alleviate menopausal symptoms and prevent postmenopausal osteoporosis. It also treats specific estrogen deficiencies.
Uses & Indications
Dosage
Adults
For HRT, typically 1 mg or 2 mg orally once daily. Dosage should be individualized to achieve symptom control with the lowest effective dose. Usually taken continuously or cyclically with a progestogen.
Elderly
No specific dose adjustment required, but use with caution due to increased risk of adverse effects. Start with the lowest effective dose.
Renal_impairment
No specific dose adjustment required in mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take the tablet orally with water, with or without food, preferably at the same time each day. If prescribed with a progestogen, follow the combined regimen.
Mechanism of Action
Estradiol binds to and activates estrogen receptors (ERα and ERβ) in target tissues, regulating gene expression and influencing various physiological processes. In HRT, it replenishes declining estrogen levels, reducing menopausal symptoms.
Pharmacokinetics
Onset
Varies; symptom relief typically observed within weeks.
Excretion
Mainly through the urine as sulfate and glucuronide conjugates.
Half life
Approximately 1-2 hours (for estradiol itself, but active metabolites have longer effects).
Absorption
Well absorbed from the gastrointestinal tract; significant first-pass metabolism.
Metabolism
Primarily in the liver via CYP3A4 to estrone and estriol, and their conjugates.
Side Effects
Contraindications
- •Known, suspected or history of breast cancer
- •Known or suspected estrogen-dependent malignant tumours (e.g., endometrial cancer)
- •Undiagnosed vaginal bleeding
- •Untreated endometrial hyperplasia
- •Previous or current venous thromboembolism (DVT, pulmonary embolism)
- •Active or recent arterial thromboembolic disease (e.g., angina, myocardial infarction, stroke)
- •Acute liver disease, or a history of liver disease where liver function tests have failed to return to normal
- •Porphyria
- •Known hypersensitivity to estradiol or any of the excipients
Drug Interactions
Corticosteroids
Estrogens may decrease the clearance of corticosteroids, potentially increasing their effects.
Thyroid hormones
Estrogens can increase thyroid-binding globulin, requiring adjustments to thyroid hormone replacement therapy.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase estradiol levels, potentially leading to increased adverse effects.
CYP3A4 inducers (e.g., carbamazepine, phenobarbital, phenytoin, rifampicin, St. John's Wort)
May decrease estradiol levels, reducing efficacy.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, breast tenderness, abdominal pain, drowsiness, and vaginal bleeding. There is no specific antidote; treatment is symptomatic and supportive.
Pregnancy & Lactation
Valestra 2 mg is contraindicated during pregnancy and lactation. It is not indicated for use during pregnancy or for the prevention of miscarriage. Small amounts of estrogens can pass into breast milk.
